Jobomschrijving

Z.org KU Leuven zoekt een aanmeldingscoördinator voor de afdeling Oudreach van het Universitair Psychiatrisch Centrum KU Leuven te campus Kortenberg.

je functie

  • je werkt binnen een mobiel multidisciplinair team gespecialiseerd in psychiatrische en psychotherapeutische zorgverlening van 65 plussers met een Ernstige Psychiatrische Aandoening (EPA)
  • je ontvangt telefonische aanmeldingen en staat in voor crisishulpverlening
  • je neemt een therapeutische rol op in het begeleiden van de individuele patiënt en het betrokken systeem én overlegt met het reeds aanwezige zorgnetwerk of helpt met het uitbouwen van een netwerk
  • je evalueert het verloop van de behandeling op regelmatige tijdstippen met het netwerk rondom de patiënt in de vorm van een zorgoverleg en bent bereid hierin een coördinerende rol op te nemen
  • je hebt een bachelordiploma in de verpleegkunde, bachelor diploma sociaal werk met afstudeerrichting maatschappelijk werk of een masterdiploma in de klinische psychologie
  • je hebt ervaring met psychiatrische en psychologische fenomenen binnen de ouderen psychiatrie (neurocognitieve aandoeningen, stemmingsstoornissen, rouw, psychose, …)
  • je kan een crisis of opnamevraag inschatten, ter plaatse gaan om zorg te verlenen en/of de oudere op weg helpen naar een veiligere situatie
  • je hebt ervaring met het werken met ouderen en hun families
  • je bent een goede organisator, je bent luistervaardig en weet goed hoofd- en bijzaken te onderscheiden
  • je zoekt actief naar oplossingen maar kan ook afgrenzen wanneer nodig
  • je neemt deel aan nieuwe initiatieven en je bent bereid je blijvend bij te scholen
  • je bezit de nodige stressbestendigheid om in crisissituaties accuraat op te treden
  • je bent collegiaal en transparant en je bent bereid om op een flexibele wijze taakafspraken te maken met de collega’s om zo het totaalpakket van opdrachten binnen de afdeling te realiseren
  • je verkiest te werken in een context van een Universitair ziekenhuis en bent bereid mee te werken aan wetenschappelijk onderzoek
  • je draagt professionaliteit en expertise mee uit naar interne en externe contacten
  • je bent mobiel en in het bezit van een rijbewijs B, verplaatsingen maken deel uit van je werk
