Position Overview:
US Career Partners is recruiting on behalf of a well-established outpatient medical center in Brooklyn, NY, seeking a compassionate, skilled, and dedicated Physical Therapist to join their team. The ideal candidate brings strong clinical knowledge, excellent communication skills, and a genuine commitment to helping patients achieve their rehabilitation goals. This role is well-suited for a PT who enjoys working directly with patients in an outpatient setting and values being part of a collaborative healthcare environment.
Schedule:
Sunday, Tuesday & Thursday: 9:00 AM – 5:00 PM Monday & Wednesday: 11:00 AM – 7:00 PM
Responsibilities:
- Provide comprehensive physical therapy evaluations and treatment to patients in an outpatient setting
- Develop individualized treatment plans based on patient needs, diagnoses, and functional goals
- Perform therapeutic exercises, manual therapy, and other appropriate physical therapy interventions
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, mobility, injury prevention, and home treatment plans
- Maintain accurate and timely clinical documentation
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care
- Help patients improve strength, mobility, function, and overall quality of life
- Follow established clinical, regulatory, and documentation standards
Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent accredited Physical Therapy degree
- Active New York Physical Therapist license
- Active New York Medicare and Medicaid provider IDs — required
- Strong clinical and assessment skills
- Excellent communication and interpersonal skills
- Compassionate, patient-centered approach
- Ability to work effectively both independently and as part of a multidisciplinary team
- Strong organizational and documentation skills
- Commitment to providing high-quality care to a diverse patient population
