Välkommen till oss där vi med våra gemensamma kunskaper bidrar till att förbättra livssituationen för patienter vi möter. Ingen dag är den andra lik, alla patienter unika och det är precis därför vi har så roligt på jobbet. I Västerviks kommun arbetar vi med hemrehabilitering och aktiverande förhållningssätt vilket innebär att vård- och omsorgspersonal arbetar tillsammans för att patienten ska bibehålla och öka sina kroppsliga och psykiska funktioner. Vi arbetar med hemrehabilitering. På riktigt! Nu söker vi en vikarie till vårt rehabteam. Dina arbetsuppgifter Hos oss blir du en del av ett engagerat team som arbetar tillsammans för att ge bästa möjliga rehabilitering till våra patienten inom den kommunala primärvården. En viktig del i arbetet är att handleda och delegera till omvårdnadspersonal för att främja ett rehabiliterande arbetssätt. Vi satsar på tidiga insatser, allt för att möjliggöra för fler att bo kvar hemma med god livskvalitet. Vi samarbetar i teamet med andra vårdgivare för att sätta patienten i centrum. En av våra inriktningar är Team Hemgång. Du kommer bedöma individens aktivitetsförmåga, kroppsfunktion samt omgivningsfaktorer. Genom att tillämpa åtgärder som aktivitetsträning, hjälpmedelsförskrivning och anpassning av miljön kommer du att stödja och främja individens förmåga att vara aktiv, delaktig och självständig. I dina arbetsuppgifterna ingår även att bedöma behov av bostadsanpassning. Din kompetens Vi söker dig som är legitimerad arbetsterapeut. För tjänsten krävs att du har ett giltigt B-körkort. Det är meriterande om du har arbetslivserfarenhet, gärna inom kommunal hälso- och sjukvård. Arbetet ställer höga krav på såväl självständighet, flexibilitet och samarbetsförmåga då förutsättningar många gånger kan förändras snabbt. Övrigt För att kunna komma i fråga för aktuell anställning i Västerviks kommun ska utdrag ur polisens belastningsregister uppvisas. Utdraget beställer du här: HVB-hem, e-tjänst | Polismyndigheten. Vänligen observera att handläggningstiden kan vara upp till 2 veckor. Du kommer att behöva visa upp ditt utdrag om du blir erbjuden en tjänst.
