Coach clinicians on safe home-based care, patient and caregiver teaching, escalation of changes in condition, and effective communication with physicians and interdisciplinary team members. Coach preceptors on consistent skills demonstration, teach-back, observation, and documentation methods. Implement assigned skill-specific remediation plans, document repeat demonstrations, and promptly escalate continued deficiencies or patient-safety concerns. Adjust instruction to the learner’s experience and scope; do not independently authorize practice beyond verified credentials or competencies.
Documentation, Safety & Collaboration
Complete competency records, observation notes, attendance, remediation outcomes, and education system entries accurately and on time. Coordinate training schedules, field observations, and access to equipment with Clinical Education, Patient Care Managers, and service-line leadership without assuming responsibility for staffing decisions. Identify recurring learning needs and recommend training updates based on observation, incident trends, and Quality & Compliance findings. Follow CMS requirements, Florida nursing requirements, applicable Joint Commission standards, infection prevention, privacy, and organizational policies. Escalate unsafe practices immediately and stop a training activity or supervised procedure when necessary for patient safety; notify the appropriate licensed/operational leader.
