Unser Kunde, eine nicht profitorientierte Spitex-Organisation im Limmattal erbringt im Auftrag der Gemeinden umfassende ambulante Pflege. Ein engagiertes Fachteam ist täglich und rund um die Uhr im Einsatz - mit dem Ziel, die Lebensqualität der Klientinnen und Klienten zu sichern und ihre Selbstständigkeit im gewohnten Umfeld zu stärken.

Stellenbeschreibung ##

  • Du bist in der direkten Pflege tätig und übernimmst bei komplexen Pflegesituationen, die Beratung und Anleitung der Kundinnen und Kunden, Angehörigen und Mitarbeitenden
  • Du nimmst Bedarfsabklärungen mit dem Instrument InterRAI vor
  • Als Fallführung bist Du verantwortlich für die Planung und Durchführung des Pflegeprozesses bei Deinen Kundinnen und Kunden
  • Du pflegst eine aktive Rolle in der Zusammenarbeit mit dem betreuenden, interdisziplinären Netzwerk und koordinierst die entsprechenden Dienstleistungen für Deine Kundinnen und Kunden

Anforderungsprofil ##

  • Du verfügst über einen Abschluss als Dipl. Pflegefachfrau HF / FH sowie idealerweise entsprechende Berufserfahrung in diesem Bereich
  • Du bist eine aufgeschlossene und flexible Persönlichkeit, die gerne auf Menschen zugeht
  • Du bringst gute Kommunikationsfähigkeit sowie sicheren Umgang mit mobiler Pflegedokumentation (Smartphone) mit
  • Du denkst vernetzt, arbeitest selbständig und verantwortungsvoll
