The posting

De vacature is als begeleider/trajecter. Dit betekent dat je kortdurende begeleidingstrajecten opneemt, inclusief handelingsgerichte diagnostische (HGD)-trajecten en ook trajecten m.b.t. verontrusting.

Binnen de domeinen psychisch en sociaal functioneren, leren en studeren en onderwijsloopbaan, ga je met de leerling zelf aan de slag maar werk je ook nauw samen met ouders en school. Intake, zo nodig psychodiagnostisch onderzoek, adviesverstrekking, zowel naar de leerling als naar de ouders en de school, zijn inherent aan deze opdracht.

Door je kennis van de sociale kaart kan je ook gericht en zo nodig verwijzen naar externe instanties.

Je maakt deel uit van een enthousiast en gedreven multidisciplinair team dat samenwerkt met zowel lagere als secundaire scholen. Je neemt op maandagnamiddag deel aan het teamoverleg waar je deelneemt aan het casusoverleg.

Ons werkgebied is groot-Aalst, Erpe-Mere en Haaltert.

· Je bent in het bezit van een diploma bachelor Sociaal Werk.

· CLB-ervaring en/of ervaring met het Buitengewoon Onderwijs strekt tot aanbeveling.

· Je hebt inzicht in het werken met kinderen en jongeren met bijzondere ondersteuningsnoden thuis en op school.

· Je hebt inzicht in de principes van het zorgcontinuüm, handelingsgericht en oplossingsgericht werken.

· Je hebt zicht op de noden van en werken met kinderen en jongeren uit kansarme milieus.

· Je kan zelfstandig werken maar functioneert ook goed en graag in een groep.

- Je werkt kwaliteitsvol (nauwkeurig, stipt, deskundig, kritisch, evaluerend, …) en resultaatsgericht .

· Je beschikt over de nodige kritische zin.

· Je bent vlot in communicatie naar cliënten en professionals, zowel schriftelijk als mondeling

· Je bent flexibel in het plannen van je werk.

· Je hebt de nodige digitale skills.

· Je bent in het bezit van een eigen wagen.
