The posting

A growing healthcare services organization is seeking an experienced Medical Director to provide clinical leadership and oversight for a large network of skilled nursing facilities throughout the New York metropolitan area. The organization is not a nursing home operator. Instead, it partners with skilled nursing facilities and senior care communities by providing specialized physiatry, pain management, and related clinical services directly within those facilities. Its physicians and healthcare professionals work alongside existing facility teams to deliver high-quality, patient-centered care focused on pain management, rehabilitation, mobility, functional improvement, and overall quality of life. The organization has an established presence across Queens, Brooklyn, and select areas of Long Island and continues to expand its clinical operations and partnerships.

The Medical Director – Physiatry & Pain Management will serve as the senior clinical leader responsible for overseeing the delivery of high-quality physiatry and pain management services across approximately 30–40 skilled nursing facility locations. This individual will provide clinical direction, mentorship, leadership, and oversight to a team of approximately 10 on-site providers while ensuring consistency, quality, and excellence in patient care throughout the organization's network.

This is a highly visible leadership position that combines clinical expertise with provider management, quality oversight, and operational leadership. The ideal candidate will have substantial direct experience working within skilled nursing facilities and a deep understanding of the unique clinical, regulatory, and operational environment of SNFs. The Medical Director will primarily provide leadership and oversight remotely, with on-site travel required only when necessary to address clinical, provider, facility, or operational needs. Candidates may be based in New York, New Jersey, or Connecticut, provided they maintain an active New York medical license and are able to travel to facilities throughout Queens, Brooklyn, and Long Island when needed.

Key Responsibilities

  • Provide overall medical and clinical leadership for physiatry and pain management services delivered across approximately 30–40 skilled nursing facility locations.
  • Provide clinical oversight and management for a team of approximately 10 on-site physicians and advanced practice providers.
  • Establish and maintain high standards of clinical care, ensuring providers consistently deliver evidence-based, patient-centered physiatry and pain management services.
  • Serve as a senior clinical resource for providers managing medically complex patients, chronic pain, rehabilitation needs, functional limitations, and other conditions commonly encountered in the skilled nursing environment.
  • Provide guidance on clinical decision-making, treatment planning, pain management strategies, rehabilitation goals, and coordination of care.
  • Monitor provider performance and clinical quality across the organization's facility network, identifying opportunities for improvement and implementing appropriate corrective or development plans when necessary.
  • Promote consistency in clinical practices, documentation, communication, and care delivery across multiple skilled nursing locations.
  • Collaborate with facility administrators, Directors of Nursing, nursing leadership, rehabilitation teams, attending physicians, and other healthcare professionals to ensure effective coordination of patient care.
  • Build and maintain strong working relationships with skilled nursing facility leadership and serve as a trusted clinical partner to facility teams.
  • Support the organization's relationships with facility partners by ensuring a high level of provider performance, responsiveness, professionalism, and clinical service.
  • Review clinical outcomes, quality indicators, documentation, and other relevant metrics to identify trends, risks, and opportunities to improve patient care.
  • Ensure providers maintain appropriate clinical documentation and adhere to applicable clinical, regulatory, and professional standards.
  • Assist with the development and implementation of clinical protocols, policies, and best practices related to physiatry, pain management, rehabilitation, and care coordination.
  • Provide ongoing education, coaching, mentorship, and professional development to the provider team as appropriate.
  • Participate in provider recruitment, onboarding, credentialing, and clinical integration as the organization continues to grow.
  • Assist with evaluating new providers and supporting the development of a strong, high-performing clinical team.
  • Travel to facilities throughout Queens, Brooklyn, and select areas of Long Island when an on-site presence is required for clinical oversight, provider support, relationship management, quality initiatives, or other organizational needs.
  • Identify and address clinical or operational issues that may impact the quality or consistency of services provided at partner facilities.
  • Work closely with executive and operational leadership to develop strategies that support clinical excellence, provider retention, organizational growth, and strong facility partnerships.
  • Stay current with developments in physiatry, pain management, rehabilitation medicine, skilled nursing care, post-acute care, and applicable healthcare regulations.
  • Serve as a visible clinical leader and ambassador for the organization's commitment to high-quality care and improved outcomes for patients in the skilled nursing environment.

Qualifications

  • Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) with appropriate postgraduate training in Physical Medicine & Rehabilitation (PM&R), Physiatry, Pain Management, or a closely related specialty.
  • Active and unrestricted medical license in the State of New York is required.
  • Candidates may reside in New York, New Jersey, or Connecticut, but must maintain active New York licensure and be able to travel to the organization's facilities throughout Queens, Brooklyn, and Long Island when necessary.
  • MUST have significant and direct experience working in skilled nursing facilities (SNFs). Candidates without meaningful SNF experience will not be considered.
  • Proven experience providing or overseeing physiatry and/or pain management services in a skilled nursing or post-acute care environment.
  • Prior experience as a Medical Director, Clinical Director, physician leader, or in another medical leadership capacity is strongly preferred.
  • Demonstrated ability to lead, mentor, and manage a team of physicians and/or advanced practice providers.
  • Strong understanding of the clinical and operational dynamics of skilled nursing and post-acute care.
  • Experience working collaboratively with nursing leadership, rehabilitation teams, facility administrators, and interdisciplinary care teams.
  • Strong clinical judgment and the ability to provide thoughtful guidance on complex patient care and pain management matters.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with providers and facility leadership.
  • Ability to balance clinical leadership responsibilities with operational priorities and organizational objectives.
  • Comfortable overseeing a geographically distributed provider team and managing multiple facilities simultaneously.
  • Willingness and ability to travel on-site to facilities throughout Queens, Brooklyn, and select areas of Long Island when needed.
  • Strong organizational skills, professionalism, accountability, and the ability to operate effectively in a growing healthcare organization.

Compensation & Benefits

  • Competitive base salary of $300,000–$350,000 annually, commensurate with experience, clinical background, and qualifications.
  • Medical Director-level leadership position with significant responsibility for clinical quality, provider performance, and patient care across approximately 30–40 skilled nursing facilities.
  • Opportunity to lead and manage a team of approximately 10 on-site physicians and advanced practice providers.
  • Opportunity to play a significant role in shaping clinical standards, provider performance, quality initiatives, and patient outcomes across a growing healthcare services organization.
  • Collaborative environment working alongside physicians, advanced practice providers, nursing professionals, rehabilitation teams, and skilled nursing facility leadership.
  • Opportunity to make a meaningful impact on the quality of physiatry, pain management, rehabilitation, and post-acute care services delivered to skilled nursing facility residents.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
