  • Complete comprehensive initial, ongoing, and as-needed assessments of patients and families — including physical, psychosocial, emotional, spiritual, and environmental factors
  • Provide skilled nursing care in accordance with the Hawaii Nurse Practice Act and hospice standards of practice
  • Assess and manage pain and symptoms using evidence-based hospice and palliative care practices

  • Ensure continuity of care through effective communication with physicians, patients, families, caregivers, and interdisciplinary team members
  • Participate in interdisciplinary group (IDG) meetings and contribute to collaborative care planning and goal setting
  • Ensure appropriate durable medical equipment, medications, supplies, and services are in place

Interdisciplinary & Supportive Care Collaboration

  • Identify psychosocial, emotional, spiritual, and bereavement needs and communicate findings to the IDG
  • Coordinate referrals to social work, chaplaincy, bereavement, and volunteer services based on patient and family needs

  • Provide anticipatory guidance, emotional support, and reinforcement of hospice goals of care
  • Teach self-care techniques to patients and caregivers as appropriate

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
