Services SFT est à la recherche de ressources pour plusieurs besoins en Soutien à domicile (SAD) au CISSS de la Gaspésie.

Horaire

  • Quart de jour
  • Temps complet (35 h/semaine)

Rôle

L’infirmier(ère) ou l’infirmier(ère) auxiliaire en soutien à domicile intervient auprès d’une clientèle en perte d’autonomie afin d'assurer des soins sécuritaires, personnalisés et de qualité dans leur milieu de vie. Il ou elle contribue au maintien de l’autonomie des usagers en collaboration avec l’équipe interdisciplinaire.

Tâches principales

  • Évaluer la condition de santé physique et mentale des usagers à domicile.
  • Planifier, prodiguer et assurer le suivi des soins infirmiers.
  • Élaborer, appliquer et assurer le suivi du plan thérapeutique infirmier (PTI), selon le champ d'exercice.
  • Participer à l’élaboration et au suivi du plan d’intervention.
  • Administrer les soins et traitements infirmiers et médicaux.
  • Effectuer l’enseignement auprès des usagers et de leurs proches.
  • Repérer les situations à risque et intervenir selon les protocoles en vigueur.
  • Collaborer avec les professionnels de la santé afin d'assurer la continuité des soins.
  • Participer à la promotion de la santé et à la prévention de la maladie.
  • Effectuer de la garde en soins palliatifs, selon les besoins du service.
  • Intervenir auprès de la clientèle desservie par les différents points de service du territoire.

Exigences

Infirmier(ère)

  • Être membre en règle de l'OIIQ.
  • Minimum 2 ans d'expérience comme infirmier(ère).
  • Expérience significative en soutien à domicile (SAD).

Infirmier(ère) auxiliaire

  • Être membre en règle de l'OIIAQ.
  • Minimum 1 an d'expérience en soutien à domicile (SAD).

Pour les deux titres d'emploi

  • Diplôme.
  • Plumitif.
  • Permis de conduire valide.
  • Certification RCR à jour.
  • Curriculum vitae.
  • Voiture obligatoire pour le mandat.
