Lieu: Services sociaux Puvirnituq et Inukjuak | Baie-James

Période : Du 28 septembre au 20 novembre 2026

Horaire: Temps complet (35h/semaine)

Quart: Jour.

Résumé des tâches :

  • L’évaluation ou la révision des besoins de l’usager .
  • L’élaboration ou la révision du plan d’intervention (PI/PII) ou du plan de service individualisé (PSI) de l’usager Faire les demandes d’hébergement.
  • Participe à l’orientation de l’usager vers le type de ressource et le type d’organisation résidentielle adéquats En collaboration avec la DEQEPE Le jumelage/pairage de l’usager avec une ressource.
  • L’intégration de l’usager dans la ressource. L’activité consiste à intégrer physiquement l’usager dans la ressource. Support à l’intégration dans le nouveau milieu de vie.
  • Faire le suivi au représentant légal ou la famille.
  • Faire une vigie sur les budgets des clients.
  • Faire le suivi au I-CLSC (Note de suivi)
  • Collaboration avec les partenaires internes et externes et référencement au besoin.
  • Collaboration à l’évaluation et réévaluation du besoin d’aide à la représentation
  • La détermination des services de soutien ou d’assistance particuliers à rendre à l’usager (instrument de classification) Faire la classification dans les 30 à 60 jours suivant le placement.
  • Élaborer l’OCCI lorsqu’il a une demande d’hébergement.
  • La mise en œuvre du PI ou du PSI de l’usager.
  • Arrimer le départ de l’usager de la ressource.
  • Collaboration dans L’application du processus de contrôle de la qualité.
  • Compléter ou mettre à jour le sommaire de personne de l’usager et la remise doit être fait dans les 72 heures suivant le placement.

Exigences:

  • Doit avoir la formation sur les ouvertures de régime de protection.
  • Minimalement 2 ans d’expérience comme TTS
  • Diplôme d'études collégiales en travail social reconnu par le ministère de l'Éducation du Québec ou son équivalence du MIFI.
  • Le remboursement des frais de déplacement se fera une fois par période d'assignation.
  • Aucun animal de compagnie n'est accepté dans les hébergements.
  • Formation pour les ouvertures de régime de protection
  • Expérience avec la clientèle nuit.
  • Doit être membre de son ordre professionnel (TS)
  • Doit posséder un permis de conduire classe 5
  • Doit être bilingue.
