Technicien(ne) en travail social – Soins à domicile à Forestville | Haute Côte-Nord

Description du poste

Services SFT recherche activement un(e) Technicien(ne) en travail social (TTS) pour combler un mandat temporaire à temps complet au sein de l'équipe des soins à domicile de la Haute-Côte-Nord, couvrant les secteurs de Forestville et Les Escoumins.

Ce mandat est idéal pour un(e) professionnel(le) autonome qui souhaite s'investir auprès d'une clientèle à domicile dans un cadre naturel exceptionnel, tout en bénéficiant des excellentes conditions de la grille des régions éloignées.

Détails du mandat :

  • Période : Du 19 octobre 2026 au 17 février 2027 (sans possibilité de prolongation)
  • Horaire : Quarts de JOUR
  • Fin de semaine : Au besoin, selon les nécessités du service
  • Orientation : Prévoir un bloc de 3 jours d'orientation en début de mandat

Responsabilités

Rattaché(e) au programme de soutien à l'autonomie des personnes âgées ou de la déficience physique (soins à domicile), vos principales fonctions seront :

  • Évaluer les besoins psychosociaux des usagers et de leurs proches aidants en visitant les milieux de vie.
  • Participer à l'élaboration, à la mise en œuvre et au suivi des plans d'intervention ou de services individualisés.
  • Apporter un soutien psychosocial, de l'écoute et de l'accompagnement aux usagers en perte d'autonomie ou en situation de crise.
  • Orienter et référer les bénéficiaires vers les ressources communautaires, institutionnelles ou de soutien matériel appropriées.
  • Collaborer étroitement avec l'équipe interdisciplinaire (infirmiers, ergothérapeutes, médecins) pour favoriser le maintien à domicile sécuritaire des usagers.

Exigences

Pour soumettre votre dossier de candidature, vous devez fournir les documents démontrant les critères suivants :

  • Détenir un diplôme d'études collégiales (DEC) en techniques de travail social (ou une équivalence reconnue).
  • Une expérience préalable en soins à domicile (SAD) ou au sein du réseau de la santé (CISSS/CIUSSS) est un atout majeur.
  • Permis de conduire valide requis (déplacements fréquents à prévoir entre Forestville et Les Escoumins).
