Description du poste

Services SFT recherche activement un(e) Travailleur(euse) social(e) (TS) autonome et engagé(e) pour combler un mandat temporaire à temps complet au sein de l'équipe des soins à domicile de la Haute-Côte-Nord, couvrant les secteurs de Forestville et Les Escoumins.

Ce mandat est une opportunité idéale pour un(e) professionnel(le) de l'intervention psychosociale qui souhaite exercer auprès d'une clientèle à domicile dans une région chaleureuse, tout en bénéficiant des excellentes conditions de la grille des régions éloignées.

Détails du mandat :

  • Période : Du 19 octobre 2026 au 17 février 2027 (sans possibilité de prolongation)
  • Horaire : Quarts de JOUR
  • Fin de semaine : Au besoin, selon les nécessités du service
  • Orientation : Prévoir un bloc de 3 jours d'orientation en début de mandat

Responsabilités

Au cœur de l'équipe du soutien à l'autonomie des personnes âgées (SAPA) et de la déficience physique, vos principales fonctions seront :

  • Évaluer le fonctionnement social, les besoins psychosociaux et le niveau de vulnérabilité des usagers directement dans leur milieu de vie.
  • Intervenir dans des situations complexes, de crise ou de vulnérabilité (perte d'autonomie sévère, maltraitance, deuil, isolement).
  • Élaborer, coordonner et appliquer des plans d'intervention psychosociale en étroite collaboration avec les proches aidants et l'équipe interdisciplinaire.
  • Accompagner les usagers et leurs familles dans les processus légaux de protection (ouverture de régimes de protection, homologation de mandats d'inaptitude, évaluations d'inaptitude).
  • Orienter et lier les bénéficiaires vers les ressources du réseau de la santé et les partenaires communautaires appropriés.

Exigences

  • Être membre en règle de l'OTSTCFQ (Ordre des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec).
  • Détenir un diplôme universitaire (Baccalauréat ou Maîtrise) en travail social.
  • Permis de conduire valide requis pour effectuer les déplacements réguliers auprès des usagers entre Forestville et Les Escoumins.
