Ben je een gekwalificeerde diëtist(e)? Bij ons begeleid je zowel gehospitaliseerde en/of ambulante patiënten met persoonlijk en wetenschappelijk onderbouwd voedingsadvies. Je ondersteunt je patiënten in hun zelfmanagement en werkt nauw samen binnen een multidisciplinair team aan kwaliteitsvolle zorg.

  • 19 uur/week (50%)

OVER DE JOB

  • Begeleiding patiënten – Je staat in voor de diëtetische behandeling en begeleiding van gehospitaliseerde en/of ambulante patiënten.
  • Behandelplannen opstellen – Je verwerkt medische en patiëntengegevens. Je vertaalt deze naar een individueel en realistisch voedingsplan overeenkomstig de noden, doelstellingen en behandeling van de patiënt.
  • Educatie rond voeding – Je geeft individuele voedingseducatie, evalueert de voortgang en stuurt de doelstellingen bij op basis van de behoeften van elke patiënt.
  • Samenwerking met artsen en andere zorgverleners – Je onderhoudt contact met patiënten en familie. Je werkt nauw samen met artsen, medisch, paramedisch en keukenmedewerkers. Je neemt actief deel aan team- en multidisciplinair overleg.
  • Kwaliteitsvolle en evidence-based diabeteszorg – Je volgt de recente richtlijnen binnen de diëtetische zorg en draagt bij aan de verdere uitbouw van kwalitatieve, wetenschappelijk onderbouwde en patiëntgerichte zorg.

OVER JOU

  • Diploma – Je beschikt over een bachelor voedings- en dieetkunde en voldoet aan de wettelijke voorwaarden om als diëtist te werken (met FOD-visum).
  • Kennis en ervaring – Ervaring als diëtist is een troef. Je kan voedingsadvies geven aan diverse doelgroepen. Je hebt zowel theoretische kennis als praktische vaardigheden.
  • Communicatief – Je rapporteert helder aan je leidinggevende.
  • Empathisch – Je luistert actief naar patiënten en houdt rekening met hun persoonlijke situatie en leefstijl.
  • Samenwerkingsgericht – Je werkt vlot samen binnen een multidisciplinair team en draagt bij aan een kwaliteitsvolle en professionele zorgomgeving.
