  • Utvrđuje potrebe bolesnika i sastavlja plan zdravstvene njege.
  • Organizira, provodi i nadzire provođenje planirane zdravstvene njege.
  • Procjenjuje uspješnost provedene zdravstvene njege.
  • Surađuje s nadležnim liječnikom i izvješćuje ga o očekivanim rezultatima plana zdravstvene njege te svim uočenim promjenama kod bolesnika.
  • Rukovodi i koordinira timom za provođenje zdravstvene njege.
  • Dokumentira sve dijelove procesa zdravstvene njege, uključujući planiranje, provedbu i procjenu.
  • Utvrđuje zadaće medicinske sestre koja sudjeluje u radu multidisciplinarnog tima.
  • Kontrolira poštivanje dostojanstva osobe u skrbi, etičkih pravila i zakonitosti rada medicinskih sestara u svom timu.
  • Educira bolesnika i njegovu obitelj o održavanju zdravstvenog stanja i sprječavanju komplikacija bolesti iz područja zdravstvene njege, uključujući osobnu higijenu, prehranu, promjenu životnih navika i korištenje pomagala.
  • Sudjeluje u donošenju odluka o nabavi pribora i opreme potrebnih za provođenje zdravstvene njege.
  • Organizira i provodi trajno stručno usavršavanje članova tima kojim rukovodi.
  • Sastavlja i otprema prijedlog za kontinuiranu zdravstvenu njegu.
  • Primjenjuje i evidentira parenteralnu, peroralnu i lokalnu terapiju, prema pisanoj odredbi liječnika koja mora sadržavati naziv lijeka, dozu, način i vrijeme primjene.
  • Primjenjuje terapiju intrakutano, supkutano, intramuskularno, lokalno i oralno prema pisanoj uputi liječnika koja mora sadržavati naziv lijeka, dozu, način i vrijeme primjene tijekom skrbi.
  • Provodi enteralnu, parenteralnu i peroralnu prehranu bolesnika.
  • Provodi ili sudjeluje u provedbi dijagnostičkih i terapijskih postupaka prema protokolima ustanova i pisanoj odredbi liječnika.
  • Uzima uzorke za laboratorijske pretrage, uključujući uzimanje uzoraka kod nepokretnih i polupokretnih bolesnika.
  • Osigurava propisno zbrinjavanje infektivnog i biološkog otpada.
  • Pristupa hitnom zbrinjavanju bolesnika do dolaska liječnika.
  • Prepoznaje granice područja svoje odgovornosti.
  • Organizira službu zdravstvene njege u hitnim situacijama, uz mogućnost delegiranja poslova na nižu razinu.

  • Provodi postupke održavanja stoma.
  • Provodi postupke palijativne skrbi i zdravstvenu njegu umirućih bolesnika.
  • Uspostavlja prvi kontakt s bolesnikom i uzima osnovne podatke o bolesniku.
  • Procjenjuje prioritete pregleda i zdravstvenih postupaka.
  • Priprema bolesnika za liječnički pregled.
  • Priprema i primjenjuje terapiju koju je odredio liječnik
  • Podučava obitelj provođenju postupaka osobne higijene kod nepokretnog, teško pokretnog, pokretnog i inkontinentnog bolesnika.
  • Surađuje s nadležnom patronažnom službom.
  • Ovlaštena za samostalno provođenje osnovnih i specifičnih postupaka zdravstvene njege u okviru stečenih vještina i pod nadzorom više razine, oslanjajući se na temeljne teorije sestrinstva.
  • Visoka razina samostalnosti u kliničkom prosuđivanju, upravljanju sestrinskim procesom, mentorstvu mlađim sestrama te preuzimanju rukovodećih uloga.
  • Establishes rapport with the patient and records all information relevant to developing the nursing care plan, in consultation with the patient where their clinical condition permits.
  • Assesses the patient’s nursing care needs and develops an individualised nursing care plan.
  • Organises, delivers and supervises the implementation of planned nursing care.
  • Evaluates the effectiveness of the nursing care provided.
  • Collaborates with the treating physician and reports the anticipated outcomes of the nursing care plan and any observed changes in the patient’s condition.
  • Leads and coordinates the nursing care team.
  • Documents all stages of the nursing process, including care planning, implementation and evaluation.
  • Defines the responsibilities of the nurse working within the multidisciplinary team.
  • Monitors respect for patient dignity and compliance with ethical standards and applicable legal requirements within the nursing team.
  • Educates patients and their families on maintaining health and preventing disease-related complications within the scope of nursing care, including personal hygiene, nutrition, lifestyle modifications and the use of assistive devices.
  • Participates in decisions regarding the procurement of supplies and equipment required for nursing care.
  • Organises and delivers continuing professional development for members of the nursing team.
  • Prepares and submits referrals for ongoing nursing care.
  • Administers and documents parenteral, oral and topical medications in accordance with written medical orders specifying the medication name, dose, route and time of administration.
  • Administers intradermal, subcutaneous, intramuscular, topical and oral medications in accordance with written medical orders specifying the medication name, dose, route and time of administration during care delivery.
  • Provides enteral, parenteral and oral nutritional support.
  • Performs or assists with diagnostic and therapeutic procedures in accordance with institutional protocols and written medical orders.
  • Collects specimens for laboratory testing, including from patients who are bedbound or have limited mobility.
  • Ensures the appropriate disposal of infectious and biological waste.
  • Initiates emergency care pending the arrival of a physician.
  • Recognises the limits of their scope of practice and professional responsibility.
  • Organises nursing services in emergency situations, delegating tasks to staff at an appropriate level of competence.
  • Documents and promptly reports all observed changes in the patient’s condition to supervisory staff.
  • Prepares for, performs and documents the disinfection and sterilisation of instruments and equipment.
  • Acts in the patient’s best interests at all times.
  • Delivers planned nursing care in the patient’s home.
  • Implements measures to prevent complications associated with prolonged immobility.
  • Provides chronic wound care in accordance with established protocols.
  • Performs urinary catheterisation in female patients.
  • Provides stoma care.
  • Provides palliative and end-of-life nursing care.
  • Conducts initial patient contact and collects basic patient information.
  • Assesses priorities for clinical assessment and care procedures.
  • Prepares patients for medical examination.
  • Prepares and administers treatment prescribed by the physician.
  • Instructs family members in providing personal hygiene care for patients who are bedbound, have severely restricted mobility, are ambulatory or have incontinence.
  • Collaborates with the relevant community nursing service.
  • Authorised to independently perform basic and specialised nursing procedures within the scope of acquired competencies and under the supervision of senior nursing staff, drawing on fundamental nursing theories.
  • Demonstrates a high degree of autonomy in clinical judgement, management of the nursing process, mentorship of junior nurses, and the undertaking of leadership responsibilities.
