Wijkverpleegkundige Ouder Amstel 24 - 32 uur per week

Bij het wijkteam Ouder Amstel van Zonnehuisgroep Amstelland krijg je de ruimte om te groeien, je kennis te delen en collega’s te ondersteunen bij complexe zorgvragen. Wat ga je doen? Als wijkverpleegkundige bij Zonnehuisgroep Amstelland ben jij de spil in het zorgproces. Jij bent verantwoordelijk voor het verpleegkundig proces, het (her)indiceren en het opstellen en uitvoeren van zorgplannen. Elke dag weer maak jij het verschil in het leven van de mensen in jouw wijk. Je doet dit met de overtuiging dat gezondheid geen doel op zich is, maar een middel om cliënten te helpen hun leven te blijven leiden zoals zij dat willen.

Samen met het team zoek je naar creatieve oplossingen voor iedere cliënt, waarbij je veel verder kijkt dan alleen het verlenen van zorg. Jij zet preventie centraal en denkt mee over de uitdagingen van de toekomst, zoals vergrijzing en personeelstekorten. Bovendien weet jij hoe belangrijk digitalisering is in de zorg en draag je actief bij aan het realiseren van de juiste zorg op de juiste plek.

Daarom is aandacht voor digitalisering in de zorg en de juiste zorg op de juiste plek brengen essentieel. Elkaar stimuleren, om de cliënt de beste zorg te geven.

Wij bieden

  • Een salaris conform CAO VVT FWG 50 € 3.502,45 tot € 4.822,50. De inschaling vindt plaats op basis van kennis en ervaring. Bovenop de inschaling in FWG 50 krijg je een (tijdelijke) arbeidsmarkttoeslag van 10 procent wanneer je volledig taak bekwaam bent;
  • Een jaarcontract van 24-32 uur (afhankelijk van jouw wensen), met uitzicht op een vast dienstverband/vast contract;
  • Een tekenbonus van in totaal € 1.000,00 bruto, lees hier de voorwaarden;
  • 8% vakantiegeld en 8,33% eindejaarsuitkering (13e maand);
  • Onregelmatigheidstoeslag en een pensioenregeling;

Wij vragen

  • Het diploma hbo-verpleegkunde en BIG-registratie alsook registratie in het kwaliteitsregister van de V&VN of bereidheid tot registratie;
  • Iemand die ondernemend, creatief, enthousiast en geduldig is met en een gezonde dosis humor;
  • Iemand die niet hiërarchisch is ingesteld, maar wel proactief en in staat is om structuur te bieden.
  • Een gezellige en ervaren collega die goed in een (multidisciplinair) team kan werken;
  • Iemand die goed past in een team dat zeer zelfstandig de werkzaamheden in de wijk organiseert, waarbij collegialiteit en betrokkenheid centraal staan.
  • Een geldig rijbewijs en eigen vervoer heeft een voorkeur.
