Als referentiepersoon dementie ben jij de ambassadeur voor mensen met dementie in onze organisatie

  • je staat in voor het bevorderen van de professionaliteit en deskundigheid van medewerkers op vlak van goede dementiezorg : organiseren van opleidingsactiviteiten op maat van alle medewerkers
  • je geeft advies aan directie en medewerkers: coachen van medewerkers, casemanager bij multidisciplinaire overleggen, je bent raadgever op vlak van een dementievriendelijke woon-en leefomgeving
  • je draagt bij aan de kwaliteit van dementiezorg binnen het woonzorgcentrum: formuleren van een visie, opstellen van een beleid, evidence based richtlijnen, invoeren van goede praktijken
  • je werkt in deze functie afdelingsoverschrijdend
  • je combineert deze functie bij voorkeur met een andere halftijdse functie als verpleegkundige of paramedicus

We zoeken een hands-on collega met een positieve ingesteldheid, een sterke betrokkenheid en een passie voor dementiezorg

  • Je bent bachelor in de verpleegkunde of bachelor in een paramedische richting
  • Bij voorkeur heb je de erkende opleiding referentiepersoon dementie gevolgd of je bent bereid deze te volgen
  • Je bent enthousiast en kan collega’s motiveren en inspireren tot kwaliteitsvolle, persoonsgerichte dementiezorg
  • Je beschikt over sterke organisatorische vaardigheden en kan overlegmomenten, werkgroepen en projecten gestructureerd coördineren
  • Je hebt een coachende houding en deelt actief jouw kennis en expertise met collega's
  • Je neemt actief deel aan professionele netwerken en zet nieuwe inzichten om in de dagelijkse praktijk
  • Je bent in staat wetenschappelijke en beleidsmatige ontwikkelingen te vertalen naar concrete richtlijnen voor medewerkers
  • Je ontwikkelt en organiseert vormings- en sensibiliseringsactiviteiten afgestemd op de noden van de organisatie
