De dienst thuisverpleging H.Hart is een erkende dienst voor thuisverpleging.

Thuisverpleging H.Hart is een lokale en onafhankelijke thuisverplegingsdienst die de toegang van de patiënt tot kwalitatieve thuisverpleging wil faciliteren.

Om deze kwalitatieve zorg uit te voeren, bieden we ook zorginhoudelijke en administratieve ondersteuning aan onze thuisverpleegkundigen.

Zorg door en voor verpleegkundigen, dat is wat telt bij ons.

  • Vertrouwd team

We hechten veel belang aan een goede band tussen de zorgverlener en patiënt.

Daarom werken we in een klein team en zal je ook na introductie op één of meerdere patiëntenrondes zelfstandig werken.

  • Detailzorg

Door een vertrouwensband op te bouwen en kennis van de patiënt te verwerven, zorgen we voor een individuele aanpak. Professionele detailzorg wordt zo mogelijk gemaakt : gericht en afgestemd op de noden van de patiënt.

  • We zorgen voor jou

Zorgzaamheid dragen we hoog in het vaandel en niet enkel bij de patiënten.

Ook als zorgverlener ben je echt belangrijk voor ons.

We ondersteunen, adviseren, coachen en voorzien opleidingen.

  • Je behaalde een bachelor of bent gegradueerd verpleegkundige.
  • Je bent sociaal vaardig. Het is voor jou dan ook vanzelfsprekend om en band te ontwikkelen

met je patiënten en hun mantelzorgers.

  • Je werkt zelfstandig. Overleggen met collega’s, contact opnemen met artsen en andere

hulpverleners? Daar schrik je niet voor terug.

  • Daarnaast kan je ook goed functioneren als teamplayer in een klein team.
  • Je hebt een groot verantwoordelijkheidsgevoel.
  • Je hebt uiteraard een passie voor zorg. Je hebt een zorgzaam karakter en je vindt integriteit

belangrijk.
