Over Zorgmed

Wij zijn een middelgrote thuiszorgorganisatie met zo’n 320 medewerkers, die alle vormen van thuis- en kraamzorg in Zuidoost-Brabant levert. Wij bieden al ruim 20 jaar verpleging, verzorging en huishoudelijke zorg in Eindhoven en omstreken. Wij staan voor betrokkenheid, veiligheid en iets goed doen voor de mensen om je heen. Wat ga je doen?

Als Coördinerend Ambulant Begeleider WMO ben je voornamelijk werkzaam bij volwassen cliënten in de thuissituatie. Je begeleidt cliënten met onder andere ASS, ADHD en/of andere psychiatrische problematiek en ondersteunt hen bij het behouden van regie over hun eigen leven.

Je werkt met een vast aantal cliënten en stelt zelfstandig je rooster samen. Je stemt de begeleiding af met verwijzers (zoals gemeenten), andere zorgverleners, mantelzorgers en collega’s. In sommige casussen werk je samen met een collega. Daarnaast bewaak je de voortgang van de begeleiding en zorg je dat deze past binnen de afgegeven indicaties en financiële kaders.

Taken en verantwoordelijkheden

  • Ambulante begeleiding bieden aan volwassen cliënten in de thuissituatie;
  • Werken aan doelen vanuit het zorg- en begeleidingsplan;
  • Coördineren en afstemmen met gemeenten, verwijzers en andere betrokkenen;
  • Aanspreekpunt zijn voor cliënten, familie en mantelzorgers;
  • Signaleren van veranderingen en hier adequaat op handelen;
  • Rapporteren en bijdragen aan de kwaliteit van zorg;

Wat levert het op?

  • Een uitdagende functie met veel vrijheid en verantwoordelijkheid;
  • Salaris conform CAO VVT (FWG 45) op basis van een 36-urige werkweek; Inschaling is afhankelijk van ervaring. Boven op je loon ontvang je een eindejaarsuitkering van 8,33%, een onregelmatigheidstoeslag, reiskosten en vakantiegeld 8%;
  • Deelname aan pensioenfonds PFZW;
  • Regelmatige bijscholingen en deskundigheidsbevordering;
  • Teamoverleggen, intervisie en casuïstiekbesprekingen;
  • Werken binnen een organisatie met weinig hiërarchie en veel aandacht voor elkaar;

Wat neem je mee?

  • Een relevant HBO-diploma;
  • Ervaring in de ambulante begeleiding;
  • Ervaring met en affiniteit voor de doelgroep;
  • Goede organisatorische vaardigheden en zelfstandigheid;
  • Je herkent jezelf in onze kernwaarden: luisteren, eerlijkheid en duidelijkheid;
  • Je werkt zelfstandig, bent communicatief sterk en straalt rust en vertrouwen uit;
  • Een rijbewijs en eigen auto;
  • Woonachtig in (de nabije omgeving van) Eindhoven.
