The posting

Проводит первичный осмотр. После обращения или получения медицинского заключения врач должен осмотреть пациента в течение двух рабочих дней. На этом этапе он собирает жалобы, анамнез (медицинский, психологический, социальный) и проводит объективный осмотр. Оценивает потребности. На основе собранной информации врач определяет, в чём именно нуждается пациент: какие тягостные симптомы (например, сильный болевой синдром) нужно купировать, какие есть социальные сложности. Разрабатывает план ведения. Вместе с пациентом (или его законным представителем) врач составляет индивидуальный план: определяет частоту визитов и звонков, распределяет задачи между членами бригады (медсестрой, социальным работником, помощником по уходу). Назначает лечение. В том числе выписывает лекарственные препараты и медицинские изделия, в том числе наркотические и психотропные средства, если они показаны. Врач также контролирует, насколько терапия эффективна и безопасна. Выполняет медицинские манипуляции на дому. Это могут быть перевязки, обработка пролежней, инъекции, установка мочевого катетера и другие процедуры, которые удобнее провести вне стационара. Осуществляет динамическое наблюдение. Врач регулярно навещает пациента (или связывается с ним), чтобы следить за динамикой: как развиваются тягостные синдромы, как работает назначенная терапия, как обстоят дела с уходом за дренажами, стомами, зондами, катетерами. При необходимости врач корректирует план. Работает с осложнениями. Если возникают побочные эффекты, нежелательные реакции или другие проблемы (в том числе возникшие после диагностических или лечебных манипуляций), врач их выявляет, оценивает и при необходимости проводит профилактику или лечение. Консультирует и координирует. Врач может консультировать пациента по вопросам дополнительной диагностики. Он организует консультации других специалистов (в том числе с применением телемедицинских технологий). Также он взаимодействует с социальными службами и другими организациями, которые помогают пациенту.

Высшее образование специалитет по одной из специальностей: Лечебное дело или Педиатрия . Подготовка в интернатуре или ординатуре по одной из ряда специальностей, например: Акушерство и гинекология, Анестезиология-реаниматология, Гастроэнтерология, Гематология, Гериатрия, Онкология, Психиатрия и другие. Повышение квалификации по вопросам оказания паллиативной медицинской помощи
