Les Centres Médico\-Sociaux ont pour mission d’aider les personnes de tous âges dépendantes ou atteintes dans leur santé à rester dans leur milieu de vie le plus longtemps possible et de soutenir les proches aidants. L’APREMADOL, avec ses 4 CMS, dessert 7 communes de l’Ouest lausannois.

Nos centres Médico\-Sociaux (CMS) offrent à la population de l'ouest Lausannois des prestations pluridisciplinaires de qualité et leurs interventions reposent sur des valeurs d’humanité et d’équité.

Afin de compléter nos équipes, nous cherchons pour notre CMS de Bussigny un·e :

Ergothérapeute (80%)

Votre mission sera de gérer des situations de réadaptation et de rééducation de nos clients de tout âge afin de préserver ainsi que de développer leur autonomie.

Vos tâches principales :

  • Evaluer les besoins en matière de prévention, de réadaptation et de rééducation des clients et, en partenariat, les objectifs occupationnels et le plan d’intervention y relatif
  • Intervenir au domicile du client afin de répondre aux objectifs tout en favorisant le maintien et le développement de ses aptitudes
  • Proposer des moyens thérapeutiques et/ou auxiliaires et les implémenter en partenariat avec les fournisseurs agréés
  • Proposer des traitements, conseiller et soutenir le client et son entourage
  • Coordonner la prise en charge du client avec l’équipe pluridisciplinaire et le réseau
  • Assurer la référence des clients qui vous sont rattachés et gérer la documentation liée à votre activité

Votre Profil :

  • Diplôme HES d’ergothérapeute ou titre jugé équivalent reconnu en Suisse
  • Intérêt pour la promotion et le développement de son domaine d’activité auprès de ses collègues
  • Une première expérience professionnelle en service de réadaptation, dans les soins à domicile ou en cabinet médical privé
  • Capacité de négociation et positionnement professionnel affirmé
  • Aptitude et intérêt à travailler en équipe interdisciplinaire et avec les partenaires des réseaux
  • Etre à l’aise dans l’accompagnement de client présentant des pathologies de santé mentale représente un atout
  • Maîtrise des outils informatiques usuels
