Nos centres Médico\-Sociaux (CMS) offrent à la population Lausannoise des prestations pluridisciplinaires de qualité et leurs interventions reposent sur des valeurs d’humanité et d’équité.

Notre bureau de transport de personnes à Mobilité Réduite (TMRL) de la région de Lausanne a pour mission de coordonner les transports pour les personnes ayant des difficultés de mobilité. Ce bureau réunit les organisations de soins à domicile de l'agglomération lausannoise (Fondation Soins Lausanne, APROMAD, APREMADOL) et Pro Infirmis.

Afin de compléter nos équipes suite au départ d'un collaborateur, les CMS de la Fondation Soins Lausanne recherche pour le 1er janvier 2027 un·e:

Ergothérapeute (H/F) =====================

Taux d'activité 20%

Contrat fixe à durée indéterminée

Votre mission

  • Enregistrer et assurer le suivi des évaluations réalisées par les CMS.
  • S’entretenir avec les évaluateurs (ergothérapeutes, infirmiers et assistants sociaux) des CMS de Lausanne et environs et de Pro Infirmis lorsque les évaluations sont incomplètes ou présentent des incohérences.
  • Valider l’attribution d’un ou de plusieurs fournisseurs de transport pour les clients à mobilité réduite.
  • Assurer chaque mois la mise à jour et la transmission des listes de réévaluation pour les trois mois à venir aux CMS et aux répondants TMRL.
  • Offrir un service d’accompagnement et de formation aux évaluateurs par une présentation régulière des prestations TMRL.
  • Offrir un service de formation aux chauffeurs intervenant dans le cadre des courses TMRL.
  • Collaborer, à titre consultatif, avec le responsable TMRL à l’amélioration du fonctionnement du Bureau TMRL en matière d’évaluations.

Votre profil et vos compétences

  • Bachelor en ergothérapie ou titre jugé équivalent (diplôme étranger reconnu par la CRS)
  • Autorisation de pratiquer ou prêt à entamer les démarches \- conditionnel à l'engagement
  • Expérience en service hospitalier de soins physiques ou de psychiatrie, service de rééducation, soins à domicile ou cabinet privé un atout
  • Intérêt marqué pour la santé communautaire
  • Capacité à collaborer en réseau et en pluridisciplinarité
  • Sens des responsabilités, capacité à s'organiser de manière autonome et à fixer des priorités
