Breve descrição do cargo

O Assistente Social atua de forma técnica e especializada nos serviços da APAE, sob supervisão e orientação do coordenador responsável por cada setor. Tem como objetivo promover o bem-estar social, a inclusão e a garantia de direitos das pessoas com deficiência e de suas famílias, por meio de atendimentos, orientações, encaminhamentos e acompanhamento sociofamiliar.

Sua atuação poderá ocorrer nos diferentes serviços da instituição, como CER IV (Auditivo, Físico, Intelectual e Visual), Educação, Assistência Social e Projeto de Educação Colaborativa, sempre integrada à equipe multiprofissional e em conformidade com as diretrizes institucionais.

Principais Atribuições

Atendimento e Acompanhamento Social

  • Realizar acolhimento e escuta qualificada de usuários e familiares.
  • Identificar demandas sociais e realizar encaminhamentos conforme necessidade e orientação da coordenação.
  • Elaborar estudos sociais, relatórios, pareceres e demais registros técnicos.
  • Acompanhar usuários e famílias, promovendo o acesso aos direitos sociais e às políticas públicas.
  • Orientar quanto a benefícios socioassistenciais, previdenciários e demais recursos disponíveis.

Atuação no CER IV - Auditivo

  • Integrar a equipe multiprofissional nos processos de habilitação e reabilitação.
  • Contribuir com informações sociais para construção do Projeto Terapêutico Singular (PTS), quando aplicável.
  • Participar de reuniões de equipe, discussões de casos e planejamento das intervenções.
  • Orientar usuários e familiares sobre direitos, benefícios e serviços da rede de atendimento.

Atuação na Educação - Educação Colaborativa

  • Apoiar estudantes com deficiência em seu processo de inclusão escolar.
  • Atuar em parceria com a equipe pedagógica, famílias e escolas da rede regular.
  • Contribuir para o fortalecimento da permanência escolar e do desenvolvimento do estudante.
  • Participar de reuniões, orientações e ações voltadas à inclusão educacional.

Atuação na Assistência Social

  • Desenvolver atendimentos individuais, familiares e em grupo.
  • Planejar e executar oficinas, grupos socioeducativos e ações de fortalecimento de vínculos.
  • Identificar situações de vulnerabilidade e risco social, propondo as intervenções necessárias.
  • Realizar visitas domiciliares, quando necessário.
  • Contribuir para o fortalecimento da autonomia, da convivência familiar e comunitária e da participação social dos usuários.

Articulação com a Rede

  • Articular ações com os serviços das políticas públicas de Saúde, Educação, Assistência Social, Trabalho e demais órgãos da rede.
  • Realizar encaminhamentos e acompanhar os usuários nos serviços da rede, quando necessário.
  • Participar, quando designado, de reuniões, fóruns, conselhos e demais espaços de articulação institucional.

Gestão Documental

  • Registrar atendimentos em prontuários físicos e/ou eletrônicos e sistemas institucionais.
  • Elaborar relatórios técnicos, estatísticos e demais documentos solicitados pela coordenação.
  • Manter a organização dos documentos sob sua responsabilidade, observando o sigilo profissional e as normas éticas da profissão.

Requisitos

  • Graduação em Serviço Social.
  • Registro ativo no CRESS.
  • Conhecimento da Política Nacional de Assistência Social, SUS, educação inclusiva e legislação relacionada à pessoa com deficiência.
  • Conhecimento em elaboração de relatórios e registros técnicos.
  • Experiência na área da saúde.
  • Conhecimento básico em informática (Pacote Office e sistemas informatizados).
