Bay Cove Human Services’ mission is to partner with people to overcome challenges and realize personal potential. Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts. Job Summary: The CCS RN/LPN is responsible for providing direct services for clients at the CCS unit in a manner that promotes clients’ safety, overall health, their effective resolution of crises, and their reintegration into community based services. Employment classification will be confirmed with HR. The Nurse coordinates with the Cape Cod Community Behavioral Health Center (CBHC) to support medication continuity, health-related assessment, referrals, and safe transitions between crisis stabilization and ongoing community-based care. Supervisory Responsibilities: None Essential Functions of Position:
Maintains effective working relationships with insurers, DMH, local community resources, and treatment/rehabilitation programs throughout the Cape Cod community.
Maintains effective communications with the pharmacy and ensures that medication systems meet all required standards for storage, documentation, administration, and disposal.
Provides direct services to clients at the CCS including medication administration, health assessments, and treatment and completes documentation as required.
Participation in daily rounds with multidisciplinary team
Coordinates admissions and discharges to and from the unit.
Ensures that safety protocols are routinely reviewed and followed consistently by shift staff (such as emergency systems; identification and remedy of facility safety hazards; security systems; lock up of sharps and any potential toxins; hazardous waste management; checks protocols, checking vitals protocols, etc).
Coordinate medication reconciliation, nursing handoffs, and health-related follow-up with Cape Cod CBHC prescribers, nursing staff, primary care partners, pharmacies, and other providers.
Complete timely nursing documentation in eHana and support shared CCS/CBHC admission, referral, discharge, and care-transition workflows.Participate in CCS/CBHC case consultation, access, quality-improvement, and care-transition activities as assigned.
Performs all other duties and projects as assigned by the supervisor
Requirements for the position:
Schedule/shift: All shifts needed
All shifts needed: day (7am-3pm), evening (3pm-11pm), overnight (11pm-7am), weekend/holiday (7a-3p, 3p-11p, 11p-7a).
Full-time, part-time/per-diem schedule for each opening.
Employment classification: Non-exempt
Massachusetts Licensed Registered Nurse, or LPN, a minimum of three years nursing experience in the delivery of behavioral health services to adults and/or children and adolescents
Current certification in CPR
Ability to work collaboratively as a member of multi-disciplinary and cross-functional teams.
Demonstrated ability to work in a pressured situation and maintain clarity, focus, judgment, and compassion
Ability to identify problems and initiate solutions to problems
Ability to work with people in crisis in a hopeful and compassionate manner and model this for others
Ability to work independently
