Job Description

The Patient Assistance Coordinator provides comprehensive post-discharge follow-up for patients evaluated across the Emergency Departments. This role serves as a critical bridge between the Emergency Department, patients, primary care providers, specialists, pharmacies, hospitals, nursing facilities, and community resources to ensure that clinically significant diagnostic results are reviewed, communicated, and appropriately addressed after the patient has left the Emergency Department.

Working in collaboration with Emergency Department providers, the ED Clinical Pharmacist, and other members of the interdisciplinary healthcare team, Patient Assistance Coordinator evaluates outstanding laboratory, microbiology, radiology, cardiac monitoring, and other diagnostic results to determine appropriate follow-up needs. The specialist facilitates timely patient notification, treatment initiation or modification, referrals, education, and connection to ongoing healthcare resources.

This position supports all four Emergency Departments, helping to promote continuity of care, patient safety, timely treatment, and appropriate follow-up for patients who may otherwise have difficulty accessing or navigating healthcare services after discharge.

Responsibilities

Conduct daily review and follow-up of outstanding laboratory, microbiology, radiology, and diagnostic results for discharged ED patients.

· Monitor blood, urine, stool, wound, sputum, CSF, tick-borne illness, COVID-19, malaria, and other designated diagnostic results through finalization.

· Review radiology addendums and Zio Patch/cardiac monitoring results requiring post-discharge follow-up.

· Identify critical or clinically significant results requiring patient notification, treatment, treatment modification, additional evaluation, or referral.

· Collaborate with ED physicians and the ED Clinical Pharmacist to determine appropriate treatment and follow-up plans.

· Notify patients or responsible parties of clinically significant results and document communication and follow-up efforts.

· Initiate established protocols for conditions such as STIs, streptococcal pharyngitis, C. difficile, and UTIs.

· Utilize certified-letter processes when required for patients with critical results who cannot be reached by telephone.

· Coordinate referrals to primary care, specialty services, cardiology, physical therapy, vestibular therapy, radiology, mammography, durable medical equipment providers, and other healthcare resources.

· Assist patients without established primary care providers in connecting with appropriate primary and specialty care.

· Communicate and transmit culture and diagnostic results to receiving hospitals, primary care offices, nursing facilities, and other appropriate healthcare organizations.

· Submit medication authorizations, clarifications, and modifications as needed and assist with medication access resources.

· Provide patient education and connect patients with appropriate community and healthcare resources.

· Monitor designated results for ED patients discharged following an ED hold or observation period.

· Track post-discharge critical results and follow-up activity to support patient safety, quality improvement, and regulatory compliance.

· Maintain accurate, timely, and confidential documentation of all follow-up activities.

· Identify opportunities to improve the consistency, efficiency, and reliability of the ED results follow-up process.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
