Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality hospice care to patients and support to their families in our 6-bed Inpatient Unit located in Wexford, PA. This is a full-time position working three (3) 12-hour shifts per week. The schedule includes rotating daylight and overnight shifts, weekends, and holidays. POSITION SUMMARY The Licensed Practical Nurse provides skilled nursing and compassionate end-of-life care to hospice patients in the inpatient setting. The LPN works collaboratively with the patient, family, physician, and hospice interdisciplinary team to provide care in accordance with the patient's individualized plan of care. The LPN plays an important role in creating a peaceful, supportive environment for patients and families during an important and often difficult time in their lives. QUALIFICATIONS
Current Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania Minimum of two (2) years of patient care experience preferred Strong clinical and assessment skills Excellent communication and interpersonal skills Strong organizational skills with attention to detail Ability to work effectively as part of an interdisciplinary team
RESPONSIBILITIES
Performs nursing assessments and monitors the patient's condition throughout each shift, reporting changes in condition to the appropriate members of the hospice team. Provides skilled nursing care and personal care, including, but not limited to:
Medication administration and monitoring Monitoring vital signs and patient symptoms Bathing, grooming, dressing, and shaving Feeding and assisting with meals Assisting with ambulation and transfers Repositioning patients and changing bed linens Assisting with elimination Providing oral, skin, foot, and ear care Light housekeeping and meal preparation as needed
Provides compassionate physical, emotional, and spiritual support to patients and families in collaboration with the hospice team and individualized plan of care. Provides appropriate support and care to patients and families at the time of death, following hospice procedures and guidelines. Completes all required electronic and/or written documentation accurately and within established timeframes. Works a rotating schedule that includes daylight and overnight shifts, weekends, and holidays as required.
