Your opportunity As a Social Worker, you'll work as part of our Allied Health team to deliver high-quality, client-centred support to older South Australians experiencing complex social, emotional and life transition challenges. You'll partner with clients, families and multidisciplinary teams to achieve positive outcomes that enhance wellbeing, dignity and independence. You’ll make an impact by:
Conducting comprehensive psychosocial assessments and developing tailored intervention plans Providing counselling, education and support across areas including mental health, adjustment, family dynamics, housing and financial concerns Supporting complex decision-making processes, including safeguarding, advocacy and future planning Collaborating with multidisciplinary teams and external providers to achieve holistic outcomes Empowering clients to maintain independence, wellbeing and quality of life through person-centred care
What you’ll bring To succeed in this role, you’ll need:
Tertiary qualifications in Social Work Membership with the Australian Association of Social Workers (AASW) Current Australian Driver's Licence National Police Check
We would also love you to bring:
Experience supporting people with complex psychosocial needs, ideally within community, health or aged care environments Strong assessment, counselling and case management skills Excellent communication and relationship-building abilities A collaborative approach and commitment to multidisciplinary care Passion for supporting older people to live well, age positively and remain connected to their community
