Als begeleider-verzorgende Envia Aalst-Ninove via Familiezorg begeleid je cliënten met hun zorg- of ondersteuningsvraag door zorg te verlenen op de verschillende levensdomeinen. Je bent aanspreekpunt in dit netwerk en biedt individuele ondersteuning of ondersteuning in groep. Je focust op het welzijn van het kind, de ouder en hun omgeving met als doel hun welzijn en gezondheid te behouden, te bevorderen of te herstellen.

Wat ga je doen?

  • Je ondersteunt kinderen, ouders en gezinnen bij hun dagelijkse zorg- en ondersteuningsvragen.
  • Je biedt psychosociale, pedagogische en praktische begeleiding op maat van het gezin.
  • Je versterkt de zelfredzaamheid en het welzijn van kinderen, ouders en hun netwerk.
  • Je werkt nauw samen met collega's en betrokken partners om kwaliteitsvolle zorg te garanderen.
  • Je signaleert veranderingen in de situatie van cliënten en stemt de ondersteuning hierop af.
  • Je ondersteunt in uitzonderlijke gevallen cliënten uit andere doelgroepen om de continuïteit van de zorg en ondersteuning te garanderen, binnen je bevoegdheid en de gemaakte afspraken.

  • Je bent een zorgzame, cliëntgerichte en verantwoordelijke collega die houdt van zelfstandig werken met een sterk team als achterban
  • Je werkt zorgvuldig, kwaliteitsgericht en met respect voor de cliënt.
  • Je bent communicatief sterk, hebt oog voor veranderingen in zorgsituaties en neemt verantwoordelijkheid op in je werk.
  • Je bent leergierig, denkt mee over verbeteringen en draagt actief bij aan een positieve teamwerking.
