The posting

De lokale coördinator is iemand met expertise op niveau van project-, proces-, strategisch en change-management.

De lokale coördinator ondersteunt de netwerkcoördinator en het netwerk in het faciliteren van een cocreatie en -organisatie van het lokale zorgaanbod via de netwerken GGZ ( mesoniveau) en deze lokale samenwerkingsverbanden ( microniveau) zodat er op klinisch niveau interdisciplinaire samenwerking gestimuleerd wordt via minstens 2 dynamische zorgnetwerken, namelijk enerzijds een buurtnetwerk met eerstelijnsactoren, en een netwerk van ambulante gespecialiseerde geestelijke gezondheidszorg met actoren die gespecialiseerde zorg leveren.

We creëren als het ware een soort brugfunctie tussen de netwerken GG en de lokale multidisciplinaire samenwerkingsverbanden.

De lokale coördinator draagt de missie en visie uit van het protocolakkoord enerzijds en anderzijds van de netwerken Noolim, Reling en Ligant. Bij het uittekenen van de opdracht staat de hulpvrager/-zoeker en zijn context centraal en dit draagt de lokale coördinator eveneens breed uit. De lokale coördinator maakt een strategisch plan op waarbij doelstellingen voor de komende twee jaar in worden vervat.

Functievereisten

  • Je bent in het bezit van een diploma (bachelor/master) in de sociale en/of humane wetenschappen.
  • Je hebt ervaring in project-, planning en organisatie-en/of changemanagement;
  • Affiniteit of kennis met de eerstelijn (gezondheid, sociaal, onderwijs) is een plus;
  • Affiniteit of kennis met de geestelijke gezondheidszorg is eveneens een plus;
  • Kennis hebben van het institutionele landschap (verdeling van bevoegdheden op federaal/gewestelijk en gemeenschapsniveau kan een troef zijn);
  • Je bent mobiel en beschikt over een eigen wagen;
  • Je blijft in voortdurend contact met partners op het terrein;
  • Je bent bereid regelmatig avonduren in te zetten, die je overdag recupereert (flexibele werkrooster volgens eigen planning).
  • Neutraliteit garanderen ten opzichte van alle partners van het netwerk. Je hanteert een open communicatie;
  • Deelnemen aan regelmatige vorming, supervisie en feedbackmomenten bij FOD Volksgezondheid.

Je kan procesmatig denken en kan bestaande processen optimaliseren;

Je werkt, gestructureerd en planmatig je taken af.

Je kan de partners achter een gezamenlijke visie te scharen;
