MBO Opleidingsniveau Als ambulant begeleider Beschermd Wonen ondersteun je cliënten met complexe problematiek bij Stichting Ontmoeting, waar duidelijkheid, openheid, eerlijkheid en nabijheid in de DOEN-cultuur centraal staan. Functieomschrijving Als ambulant begeleider Beschermd Wonen begeleid je cliënten met complexe problematiek bij uiteenlopende hulpvragen, binnen het team in Apeldoorn. Je ondersteunt mensen met een WLZ- of WMO-indicatie, waarbij je langdurig een vertrouwensband opbouwt door te schakelen tussen praktische ondersteuning en verdiepende gesprekken. Samen met collega's bied je 24-uurs zorg en werk je aan stabilisatie, herstel en emotieregulatie, waarbij dossiervorming en samenwerking met partners centraal staan. De functie vraagt om zowel zelfstandigheid als teamgericht werken, ondersteund door de SRH-methodiek. Jouw werkzaamheden zullen zijn: - Begeleiden van cliënten met complexe problematiek op basis van WLZ- of WMO-indicatie. - Opbouwen van een langdurige relatie met cliënten om stabiliteit en herstel te bevorderen. - Bieden van praktische ondersteuning, voeren van verdiepende gesprekken en ondersteunen bij emotieregulatie. - Uitvoeren en bijhouden van dossiervorming en rapportages. - Samenwerken met collega's en externe partners voor optimale begeleiding. - Signaleren van knelpunten en ondernemen van actie in complexe situaties. - Werken volgens de SRH-methodiek en bijdragen aan een open en veilige werksf...
