Dit wordt jouw functie Als ambulant hulpverlener jeugdzorg begeleid je j​ongeren tot 21 jaar met uiteenlopende problemen. Deze problemen kunnen bijvoorbeeld zijn ontstaan vanwege gedrags- of ontwikkelstoornissen, vaak gecombineerd met problemen van de ouders. Je coördineert het hele begeleidingstraject van de j​ongere, en je werkt hierbij intensief samen met hulpverleners en jeugdzorginstanties. Dat kan voor een korte tijd zijn, maar ook langduriger. Bij de start van een traject breng je de hulpvraag van de j​ongeren en hun (sociale) omgeving in kaart. Aan de hand van de hulpvraag stel je doelen op voor het gezin en de j​ongere, die je duidelijk vastlegt in een begeleidingsplan. Daarbij schakel je waar mogelijk reguliere hulp in en waar nodig specialistische hulp. Ook begeleid en ondersteun je de ouders tijdens het hulpverleningstraject. Het doel is dat j​ongeren en gezinnen uiteindelijk zelf hun doelen behalen. Je werkt veelal zelfstandig en bent verantwoordelijk voor jouw cliënten; je hebt een vaste caseload. Daar waar nodig kun je casussen bespreken in multidisciplinaire overleggen. Elke situatie is anders en vraagt een andere aanpak. Daarom is je werk als ambulant hulpverlener jeugdzorg heel afwisselend! Als Ambulant Hulpverlener Jeugdzorg ga je

- Intakegesprekken voeren;

- Huisbezoeken doen, je komt dus bij de gezinnen thuis;

- Begeleidingsplannen maken aan de hand van de hulpvraag van de j​ongere of het gezin;

- Passende hulpverlening in gang zetten wanneer dit nodig is;

- Overleggen met collega’s over jouw caseload en hoe je de j​ongeren en hun ouders zo goed mogelijk kunt ondersteunen om hun doelen te behalen.
