Begeleider LVB+ - St. Anna in Heel Dienstverband 20 - 36 uur per week Opleidingsniveau MBO niveau 4 Standplaats Heel Werkvelden Gehandicaptenzorg Beschik jij over de deskundigheid en ervaring om met een complexe doelgroep te werken? Sta jij stevig in je schoenen, denk je in mogelijkheden en kun je verder kijken dan het getoonde gedrag? Dan zijn wij op zoek naar jou! Het aantal uren is in overleg te bepalen. Over de functie Je werkt met cliënten binnen de doelgroep LVB +. Deze cliënten vragen om een zo normaal mogelijk leven met de beperkingen die zij hebben. Ze zijn gericht op ontwikkeling en zelfontplooiing. Door de bijkomende gedragsproblemen is er een grote behoefte aan structuur en intensieve begeleiding. Als begeleider bied jij deze structuur en help je de cliënten met het dagelijks leven en de uitdagingen die zij hierin tegenkomen. Vaak hebben zij een IQ tussen de 50 en 85 en een duidelijk beperkt sociaal aanpassingsvermogen. Je bent daarnaast persoonlijk begeleider van (in de meeste gevallen) twee cliënten. Samen met hen, de wettelijk vertegenwoordiger en de behandelaar zorg je voor het tot stand komen van het zorgplan. In het zorgplan is de ondersteuningsvraag van de cliënt omschreven. Deze wordt als richtlijn gebruikt voor de manier van begeleiden. Tevens onderhoud je contact met verwanten, behandelaars, artsen en overige personen die betrokken zijn bij de cliënt. Je werkt in wisselende diensten, inclusief slaapdiensten. Bekijk het filmpje , waarin collega's en bewoners je graag meer vertellen over het werken met deze doelgroep. Over de doelgroep Je gaat in het kernteam werken voor de woningen aan de Schoolhof, op het terrein van St. Anna. De diversiteit van cliënten binnen deze groepen vraagt dat je gemakkelijk kunt schakelen tussen verschillende begeleidingsstijlen.
