Op het Avantis College zijn we per direct op zoek naar een Begeleider Passend Onderwijs. Wat ga je doen? Op het Avantis College zijn wij ons ondersteuningsteam aan het uitbreiden en daarom zijn we op zoek naar een Begeleider Passend Onderwijs. Het betreft een tijdelijke aanstelling voor het schooljaar 2026/2027, met uitzicht op een vast dienstverband. In deze functie speel je een centrale en zelfstandige rol in het vormgeven en uitvoeren van passend en inclusief onderwijs binnen zowel het VMBO als de ISK. Je draagt zelfstandig casuïstiek, adviseert collega's en fungeert als verbindende schakel tussen leerling, ouders, school en externe partners. Je levert een inhoudelijke bijdrage aan passend en inclusief onderwijs. Jouw werkzaamheden: • Opstellen, bewaken en evalueren van OPP's en ondersteuningsarrangementen • Analyseren van leerlinggegevens en vertalen naar concrete plannen van aanpak • Regie voeren op ondersteuningstrajecten en zorgdragen voor tijdige evaluaties • Coördineren van ondersteuningsroutes, doorstroom en eventuele doorverwijzingen • Organiseren en voorzitten van multidisciplinaire overleggen • Adviseren van mentoren, docenten en management over ondersteuningsbehoeften • Onderhouden van contacten met ouders en externe hulpverleningsinstanties Het ondersteuningsteam bestaat uit een ondersteuningscoördinator, een orthopedagoog, een leerlingondersteuner en met jou erbij een BPO'er.
