En wil je meebouwen aan toekomstgerichte, netwerkgerichte ggz binnen de Dichtbijteams van GGZ Rivierduinen? Dan is dit jouw kans. In september start GGZ Rivierduinen in Gouda met een nieuwe manier van werken. Binnen onze Dichtbijteams brengen we ambulante psychiatrie, specialistische poliklinische zorg, ouderenpsychiatrie en jeugdexpertise samen. We brengen de zorg dichter bij de cliënt en organiseren deze integraal, herstelgericht en in verbinding met het netwerk. Voor het Dichtbijteam Gouda zoeken we een Casemanager die makkelijk verbinding maakt en daadkrachtig is. Geen dag is hetzelfde. Het ene moment ben je bij een cliënt thuis, het volgende moment overleg je met een huisarts, gemeente of andere ketenpartner. Daarna bespreek je met collega's wat er nodig is om de behandeling verder vorm te geven. Dit ga je doen als casemanager: - Het voeren van herstelondersteunende intakegesprekken om beeld te krijgen van de client en diens hulpvraag; - Opstellen, uitvoeren en evalueren van herstelplannen; - Zorgdragen voor zorgcoördinatie binnen jouw caseload; - Contact onderhouden met hulpverleners, naasten en maatschappelijke partners; - Zorgdragen voor volledige en overdraagbare verslaglegging en rapportage; - Actief bijdragen aan teamontwikkeling en inhoudelijke doorontwikkeling van het Dichtbijteam; - Samenwerken binnen een Dichtbijteam met ongeveer 30 collega's.
