Over de functie Als Consulent Jeugd ben je verantwoordelijk voor het beoordelen van hulpvragen, coördineren van zorg en het bewaken van de voortgang binnen het sociaal domein. Je werkt zelfstandig aan complexe casuïstiek, signaleert onveiligheid en werkt samen met gezinnen, ketenpartners en collega's. Je draagt bij aan beleidsontwikkeling en neemt deel aan overleggen en intervisie, met focus op kwaliteit en continuïteit van jeugdhulp. Wat je gaat doen - Beoordelen van hulpvragen voor geïndiceerde jeugdhulp volgens Jeugdwet en gemeentelijke regelgeving - Coördineren van passende zorg en bewaken van de voortgang - Monitoren en evalueren van ingezette hulp en toezien op tijdige afschaling - Samenwerken met gezinnen, ketenpartners en vrij-toegankelijke ondersteuning - Signaleren, beoordelen en aanpakken van (vermoedens van) onveiligheid, uitvoeren van risicotaxaties en veiligheidsplannen - Toetsen van toewijzingsplannen aan wettelijke kaders en richtlijnen - Zorgdragen voor nauwkeurige en verantwoorde dossiervorming - Adviseren van collega's binnen het sociaal domein over jeugdhulp en veiligheid - Bijdragen aan beleidsontwikkelingen en complexe veiligheidscasuïstiek - Deelnemen aan overleggen en intervisie - Samenwerken met externe partners zoals scholen, gezondheidszorgprofessionals en jeugdzorginstanties Functie-eisen - Aantoonbare afgeronde opleiding op minimaal HBO niveau - Geldige SKJ-registratie - Minimaal 2 jaar aantoonbare werkervaring als Jeugdconsulent binnen een gemeentelijke organisatie - Niet woonachtig in de gemeente Midden-Drenthe - Rijbewijs B - Aantoonbare afgeronde opleiding op minimaal hbo niveau in de richting van Social Work of pedagogiek (pré) - Minimaal 5...
