Je schakelt daarbij nauw met inwoners, gezinnen en ketenpartners om veiligheid, perspectief en passende ondersteuning te organiseren. Je houdt overzicht in dynamische situaties en weet risico’s tijdig te signaleren en te vertalen naar concrete acties. Daarnaast leg je voortgang, afspraken en resultaten zorgvuldig vast en stuur je bij wanneer de situatie daarom vraagt. Wat ga je doen - Je realiseert samen met de inwoner, het gezin en betrokken ketenpartners duurzame en aantoonbare verbetering van de veiligheid en de leefsituatie van inwoners met complexe en meervoudige problematiek. - Je signaleert, analyseert en beoordeelt veiligheidsrisico’s tijdig. - Je stelt voor complexe casuïstiek een concreet en passend veiligheidsplan op, voert dit uit en evalueert het periodiek. - Je organiseert noodzakelijke zorg en ondersteuning tijdig en passend, met specifieke aandacht voor jeugd en het gezinssysteem. - Je laat betrokken professionals en ketenpartners, zoals Veilig Thuis, politie en Jeugdbescherming, effectief samenwerken en hun verantwoordelijkheden nakomen. - Je betrekt het sociale netwerk en het gezinssysteem waar mogelijk actief bij het realiseren van veiligheid en perspectief. - Je stelt vast welke duurzame hulpverlening en ondersteuning nodig is om terugval en escalatie te voorkomen. - Je legt voortgang, gemaakte afspraken, risico’s en resultaten volledig en zorgvuldig vast. - Je stuurt bij en schaalt op bij veranderende of acute omstandigheden wanneer de veiligheid daarom vraagt. Eisen - Je hebt een aantoonbaar afgeronde opleiding op minimaal hbo-bachelor niveau. - Je hebt aantoonbare werkervaring in de rol van consulent met complexe zorg- én veiligheidscasuïstiek. - Je beschikt over een geldige SKJ registratie. - Je hebt aantoonbare werkervaring met maken van veiligheidsplannen.
