[email protected] We zoeken vrijwel per direct een (huis)arts voor 20-31 uur per week. • Je levert huisartsenzorg op maat voor een kwetsbare en diverse doelgroep, met ruimte om écht het verschil te maken. • Je draagt bij aan de gezondheid en het welzijn van dak- en thuisloze patiënten, vaak met complexe verslavings- en psychiatrische problematiek. • Je werkt in een betrokken en professioneel team, waar samenwerking, collegialiteit en flexibiliteit centraal staan. De functie In de functie van (huis)arts voor de praktijk Dr. Valckenier lever je huisartsenzorg aan voornamelijk dak- en thuisloze patiënten, vaak met complexe verslavings- en psychiatrische problematiek. Je ontmoet een grote diversiteit aan mensen, waaronder zorgmijders, en werkt laagdrempelig op de plekken waar zorg nodig is. De huisartsenzorg die je biedt is inhoudelijk gelijkwaardig aan reguliere praktijken, maar de organisatie is volledig afgestemd op deze doelgroep. Je hebt meer tijd voor patiënten, overlegt intensief met ketenpartners en levert bemoeizorg aan mensen die zelf geen overzicht hebben. Je draait spreekuren op Geïntegreerde Voorzieningen (GGD-locaties voor verslavingsbehandeling) en opvanglocaties van het Leger des Heils of HVO Querido, altijd samen met verzorgenden of verpleegkundigen. Elke werkdag is anders: je combineert spreekuren met het bezoeken van zorgwekkende patiënten, het coördineren van medische zorg in de stad en deelname aan multidisciplinair overleg. Je verwerkt post, reageert actief op signalen over patiënten en superviseert doktersassistenten (en basisartsen).. Het werk vraagt schakelen, samenwerken en anticiperen, terwijl je steeds tijd maakt voor een luisterend oor en persoonlijke aandacht.
