In ons Wijkgezondheidscentrum De Vlier vind je een dynamisch multidisciplinair team van onder meer huisartsen, kinesisten, verpleegkundigen en een zorgkundige, een psychisch en sociaal ondersteuner, een gezondheidspromotor, administratieve medewerkers en een coördinator.

We bieden kwaliteitsvolle, toegankelijke eerstelijnszorg aan met een focus op preventie en integrale zorg voor elke patiënt. Onze multidisciplinaire aanpak steunt op een ruime expertise van collega’s (diabeteseducator, gezondheidspromotor, samenwerking ELP, artsen, verpleegkundigen, ...). Twee artsen zijn praktijkopleider.

Als artsenploeg werken we evidence-based, met regelmatig overleg zowel binnen het artsenteam als multidisciplinair in het centrum en extramuraal met andere diensten. We kunnen rekenen op een sterke administratieve ondersteuning/ontlasting van zorgverstrekkers, waardoor wij de focus op het medische luik kunnen richten. Er wordt steeds op afspraak gewerkt, huisbezoeken zijn gezien onze jonge populatie beperkt.

Wat hebben wij onder meer gerealiseerd waarop we bijzonder trots zijn! Een samenwerking met tandartsen binnen het uniek mondzorgproject Tand-em. Een zwangerschapscentrum De Wase Kiem opgestart vanuit ons centrum. Beweegsessies in groep met ondersteuning van de kinesitherapeuten. En zoveel meer …

  • Je bent gemotiveerd om ook te werken met een kansarme en multiculturele patiëntenpopulatie (verschillende nationaliteiten, culturen, godsdiensten,...) waaronder ook een grote groep met extra kwetsbaarheden (anderstaligheid, financiële uitdagingen, kansarmoede...)
  • Je bent geaccrediteerd huisarts en staat in voor de huisartsen- consultaties en huisbezoeken volgens de EBM met aandacht voor de levensdoelen van de patiënt (‘goal-oriented care’). Registratie en bijhouden medisch dossier (GMD, EMD en GMD+). Aanbevelingen van E-Health opvolgen, m.n. uploaden van Sumehrs en voorschriften via Recip-e.
  • Je neemt graag een actieve rol op bij preventieactiviteiten, de beleidsontwikkeling van het centrum en het multidisciplinair overleg met interne en externe partners met oog op een integrale zorgverlening.
