HZW Programmamanager Chronische Zorg (32 uur) - - Kantoor ( Wormer ) - € 4.766 - € 6.589 per maand - Kantoor Omschrijving Al meer dan twintig jaar vormt ketenzorg een stevige basis voor de begeleiding van mensen met een chronische aandoening. Mensen met bijvoorbeeld diabetes, COPD of een verhoogd risico op hart- en vaatziekten krijgen vanuit de huisartsenpraktijk langdurige begeleiding, meestal van de huisarts en de praktijkondersteuner. Deze zorg is vooral per aandoening georganiseerd in afzonderlijke zorgprogramma's. Die aanpak heeft veel gebracht, maar past steeds minder goed bij de zorg van nu. Steeds meer mensen hebben meerdere aandoeningen tegelijk en persoonlijke omstandigheden die invloed hebben op hun gezondheid. Tegelijk neemt de vraag naar chronische zorg toe en zijn zorgprofessionals schaars. Om ook in de toekomst iedereen passende zorg te kunnen bieden, is een andere manier van organiseren nodig. HZW wil daarom toewerken naar één samenhangende en persoonsgerichte aanpak. De patiënt krijgt meer regie, met het huisartsenteam als deskundige partner die de hele mens ziet. Positieve Gezondheid en het andere gesprek ondersteunen deze beweging: breder kijken dan de aandoening en bespreken wat voor iemand belangrijk is en wat iemand zelf kan en wil. We zoeken geen beheerder van bestaande programma's, maar een programmamanager die deze vernieuwing samen met huisartsenpraktijken en regionale partners vormgeeft. Jouw opdracht De programmalijn Chronische Zorg is het geheel van projecten, afspraken en verbeteractiviteiten waarmee HZW de chronische zorg in de regio ondersteunt. In 2027 zetten we daarin een volgende stap: de huidige programma's blijven een belangrijke basis, maar worden beter met elkaar verbonden. Zo kan de zorg meer worden afgestemd op de combinatie van aandoeningen, wensen en mogelijkheden van de patiënt. Jij leidt deze ontwikkeling.
