Je werkt aan complexe vraagstukken waarbij je snel schakelt, regie houdt en overzicht creëert. Daarbij stem je af met zorgaanbieders, beleidsadviseurs en collega’s om tot een passende aanpak te komen. Je zorgt dat de ondersteuning goed wordt gecoördineerd en dat afspraken zorgvuldig worden vastgelegd. Vanuit jouw expertise draag je bij aan passende jeugdhulp en maatwerk voor gezinnen. Wat ga je doen - Je gaat in gesprek met de inwoner, ouders én jongere om de vraag helder te krijgen. - Je stimuleert en faciliteert ouders en jongere om regie in eigen hand te nemen en/of te houden. - Je verkent samen met de ouders en jongere de eigen mogelijkheden, die van het persoonlijke netwerk en van de sociale omgeving. - Je helpt een plan van aanpak te maken met het wenselijke resultaat en tijdpad. - Je zet jouw expertise en ervaring in om zo vroeg mogelijk de juiste ondersteuning te bieden. - Je coördineert de ingezette ondersteuning en stelt deze zo nodig tussentijds bij. - Je bent de specialist op het jeugdhulpaanbod en staat in contact met zorgaanbieders. - Je geeft vanuit je dagelijkse praktijk input aan de beleidsadviseurs over kwalitatief goede en efficiënte voorzieningen in de jeugdhulp. - Je spart met je collega’s om gezamenlijk tot de juiste aanpak te komen en houdt de regie waar dat nodig is. - Je zorgt dat alles goed vastgelegd wordt en geeft opdrachten aan organisaties die maatwerkvoorzieningen kunnen leveren. - Je werkt onder leiding en toezicht van de Coördinator Team voor Elkaar. Eisen - Je hebt een aantoonbaar afgeronde opleiding op minimaal hbo bachelor niveau. - Je beschikt over een geldige SKJ-registratie en het registratienummer staat expliciet vermeld in je cv. - Je hebt minimaal 3 jaar aantoonbare werkervaring in de afgelopen 10 jaar als jeugdconsulent bij een gemeentelijke organisatie. - Je bent uiterlijk beschikbaar per 19 oktober 2026 voor..
