regio Apeldoorn / Deventer 28-32 uur Wij zoeken een Maatschappelijk werker met ervaring in het ambulante werk, specifiek met cliënten uit de WMO, WLZ en FZ. Jij hebt ervaring met complexe situaties, zoals dubbel- of triple-diagnostiek, en een scherp inzicht in de sociale kaart. Je hebt een duidelijke visie op de kwaliteit van hulpverlening en casusregie, waarbij je een overstijgend perspectief hanteert. Je weet het grotere plaatje te zien, zonder het belang van de cliënt uit het oog te verliezen. Als ondernemende en actieve professional heb je oog voor het samenspel tussen het belang van de cliënt, het team en de organisatie. Je hebt een oprechte interesse in de cliënt en benadert iedere situatie met een nieuwsgierige en oplossingsgerichte houding. Je hebt uitstekende communicatieve vaardigheden en de discipline om registraties en verslaglegging zelfstandig en effectief te verzorgen, altijd met oog voor kwaliteit. wat wij bieden Het dienstverband omvat 28 tot 32 uur per week, met een startdatum in overleg. Je krijgt in eerste instantie een contract voor de duur van één jaar. Bevalt het van beide kanten? Dan gaan wij graag met je verder. Je wordt ingeschaald in schaal 8/9 van de cao Sociaal Werk, op basis van leeftijd en ervaring. Je kunt rekenen op: - een loopbaanbudget voor jouw professionele ontwikkeling - een reiskostenvergoeding voor woon-werkverkeer en dienstreizen - een pensioenregeling via PFZW - vakantiegeld én een eindejaarsuitkering (individueel keuzebudget) wat jij meebrengt - een relevant hbo diploma (Social Work/SPH) - minimaal 3 tot 5 jaar ervaring in het ambulante werk (WMO/WLZ/FZ) - een rijbewijs en een auto - digitale vaardigheden - ervaring met ketensamenwerking - de ambitie om jezelf verder te ontwikkelen word jij onze nieuwe collega?
