Je helpt ouders groeien in hun zelfvertrouwen en zelfredzaamheid, zodat zij stap voor stap zelfstandig de zorg voor hun kindje kunnen dragen. Je houdt je onder andere bezig met: - begeleiden en ondersteunen van gezinnen in de thuissituatie; - observeren van de gezondheid en ontwikkeling van baby en verzorgers; - signaleren van risico's of veranderingen en indien nodig schakelen van andere hulpverleners; - geven van voorlichting en advies over veiligheid, hechting, verzorging en opvoeding; - opstellen, uitvoeren en evalueren van ondersteuningsplannen; - activeren van het sociale netwerk rondom het gezin; - onderhouden van contact met betrokken zorgverleners en ketenpartners. Samen maken we het verschil Je maakt onderdeel uit van het team BabythuisZorg binnen Kraamzorg en werkt nauw samen met collega's, verloskundigen, jeugdgezondheidszorg en andere ketenpartners.
