We zijn op zoek naar een persoonlijk begeleider! Wat ga je doen? Je bent verantwoordelijk voor het begeleiden van cliënten (ouders en kinderen) met een intensieve ondersteuningsvraag op psychosociaal en pedagogisch gebied. Je begeleidt de ouders bij de opvoeding van hun kinderen met het doel het vergroten/behouden van de zelfredzaamheid en het structureren van het dagelijks leven. Binnen de OuderKind locaties heb je te maken met multi-probleem gezinnen en werk je nauw samen met het netwerk en met diverse interne -en externe disciplines. Je observeert de cliënt-/gezinssituatie en analyseert interacties tussen gezinsleden, bespreekt deze en geeft feedback. Je werkt samen met de ouder aan het versterken van opvoedvaardigheden. Je stelt in samenwerking met de cliënt het gezinsplan op. Complexe en/of onveilige situaties worden multidisciplinair besproken. ASVZ werkt met de Triple C -methodiek en daarnaast wordt er binnen de OuderKind locaties met de methodiek HouVast gewerkt. Samen met het gezin ga je voor 100% en vul je aan waar nodig. Je bent verantwoordelijk voor administratieve handelingen, neemt deel aan overleggen en zorgt voor een goede overdracht van informatie. In je werk word je aangestuurd en ondersteund door een meewerkend teamleider en een orthopedagoog. Je werkt in een onregelmatig dienstenpatroon van 24-uurszorg verdeeld in dag, -avond, -slaap, -en weekenddiensten. Wie zoeken we?
