Je neemt het stokje over van een enthousiaste collega die in september/oktober met pensioen gaat. Wij zijn een enthousiast team van 2 huisartsen, 1 vaste waarnemer, 2 doktersassistentes, 1 praktijkmanager, 1 POH-S (waarvoor deze vacature), 1 POH-GGZ en 1 POH-jeugd. Je komt terecht in een warm team waarin samenwerking en kwaliteit van zorg centraal staan. Als POH-S ben je verantwoordelijk voor de begeleiding en behandeling van patiënten met chronische aandoeningen. Als praktijkondersteuner in onze praktijk krijg je veel ruimte om alle facetten van het vak uit te oefenen. - Astma/COPD • Ouderenzorg - Een betrokken en gezellige collega • Een afgeronde opleiding tot praktijkondersteuner somatiek - Goede beheersing van de Nederlandse taal en een redelijk tot goed niveau van de Engelse taal - Goede communicatieve en organisatorische vaardigheden en zelfstandigheid • Je staat open voor innovatie en denkt graag mee over verbeteringen in de huisartsenzorg - Je hebt affiniteit met leefstijl en zoekt een plek om jezelf verder te ontwikkelen. - Een collegiale werkomgeving, we werken in een klein team met korte lijntjes. - Een fijne combinatie tussen zelfstandigheid met tegelijkertijd een mogelijkheid om laagdrempelig overleg te hebben.
