Je voert medische controles uit, begeleidt patiënten bij leefstijlveranderingen en geeft advies over medicatiegebruik. Daarnaast draai je spreekuren, waarbij je intakegesprekken voert, anamneses afneemt en medische handelingen verricht. Je werkt nauw samen met huisartsen en andere zorgverleners binnen ons multidisciplinaire team. Hierbij maak je gebruik van digitale tools zoals Medicom om de zorg efficiënt en patiëntgericht te organiseren. Je krijgt de ruimte om mee te denken over verbeteringen in zorgprocessen en kennis te delen met collega's. Samen zorgen we voor toegankelijke, kwalitatieve zorg in een moderne praktijkomgeving. Wat we je bieden - Salaris tussen €3.684 en €4.832 per maand, afhankelijk van opleiding en ervaring, volgens CAO Huisartsenzorg schaal 7. - Tijdelijk contract, specifiek voor de maandagen/vrijdagen. (andere mogelijkheden in overleg! - Parttime functie van 8 tot 9 uur per week, ideaal te combineren met andere werkzaamheden. - Een plek binnen een innovatieve huisartsenpraktijk waar digitalisering en patiëntgerichte zorg centraal staan. - Toegang tot een betrokken en multidisciplinair team, met ruimte voor samenwerking en kennisdeling. - Aandacht voor vitaliteit en duurzame inzetbaarheid, inclusief een gezonde werk-privébalans en gratis vers fruit op het werk.
