Dit ga jij doen Als Procesbegeleider Jeugd werk je bij de gemeente Oosterhout binnen het sociaal team uitvoering. De focus van deze opdracht ligt op het voeren van regie op complexe (veiligheids)casuïstiek. Je verdiept je in de persoonlijke situatie van jeugdigen en hun gezinnen en bent in staat zowel enkelvoudige als zeer complexe vraagstukken op te pakken. Het sociaal team is volop in ontwikkeling en streeft naar een integraal team waarbij de procesbegeleider jeugd de spil kan zijn voor een inwoner. Dit vraagt specifieke jeugdkennis, maar ook nieuwsgierigheid naar andere domeinen zoals Participatie, WMO en schuldhulpverlening. Omdat het gaat om een pilot (POC) draag je ook actief bij aan het verbeteren van de werkwijze. - Je verdiept je in de persoonlijke situatie van de jeugdige en zijn of haar gezin; - Bij het eerste contactmoment verheld je de vraag en ga je op zoek naar de vraag achter de vraag; - Met de verzamelde gegevens maak je een analyse en stel je samen met het gezin een plan van aanpak op; - Indien inzet nodig is, zorg je voor de beschikking en een warme overdracht naar de desbetreffende hulpverlener; - Je houdt de regie over het proces en sluit aan bij evaluaties waar nodig; - Bij afloop van de beschikking evalueer je gezamenlijk de situatie en bekijk ...
