Menu Projectleider wonen, welzijn en zorg Over de functie Bij Alwel geloven we dat een thuis meer is dan een woning. Het gaat om prettig wonen, mee kunnen doen in de buurt en de ondersteuning krijgen die nodig is. Daarom maken we met gemeenten en zorg- en welzijnsaanbieders afspraken over de juiste zorg en ondersteuning voor woningzoekenden die b.v. uitstromen uit Beschermd Wonen of de Maatschappelijke Opvang. Of voor senioren die langer zelfstandig thuis (blijven) wonen, ondanks een zwaardere zorgvraag. Daarnaast ontwikkelen we vernieuwende woonconcepten waarin wonen, welzijn en zorg samenkomen voor mensen die daar extra behoefte aan hebben. Als Projectleider Wonen, Welzijn en Zorg speel jij hierin een sleutelrol. Jij zorgt ervoor dat goede ideeën werkelijkheid worden. Je brengt gemeenten en zorg- en welzijnspartners bij elkaar en vertaalt maatschappelijke vraagstukken naar concrete afspraken en woonoplossingen. Je zoekt hierbij nadrukkelijk de verbinding met collega's van andere afdelingen en neemt hun input mee. Van de eerste planvorming tot de daadwerkelijke huisvesting en/of ingebruikname geef jij richting aan projecten die écht verschil maken in het dagelijks leven van bewoners. Je werkt aan het passend huisvesten in de wijk van bewoners met een ondersteuningsvraag en maakt daarvoor afspraken. Je evalueert en stuurt bij waar nodig. Je werkt niet op casusniveau, maar neemt de ervaring van het gebiedsteam mee in het gesprek met de netwerkpartners. Daarnaast werk je aan nieuwbouw-, renovatie- en transformatieprojecten waarin leefbaarheid, gemeenschapsvorming en passende ondersteuning centraal staan. Daarbij houd je overzicht, verbind je verschillende belangen en zorg je dat plannen niet alleen op papier kloppen, maar ook in de praktijk werken. Zo bouw je mee aan inclusieve woonomgevingen waar mensen zich thuis voelen, elkaar ontmoeten en zo zelfstandig mogelijk kunnen wonen. Alwel als buurtcorporatie Fijn wonen, daar werke...
