Kantoor Groningen of Utrecht Regisseur in-, door- en uitstroom (32-36 uur per week) Functieomschrijving Wil jij jouw ervaring in de zorg inzetten in een functie waarin cliëntcontact, complexe casuïstiek en regie samenkomen? Als Regisseur in-, door- en uitstroom begeleid je trajecten van mensen die bij Fokus willen wonen, willen doorstromen naar een andere Fokuswoning of uitstromen. Je onderzoekt wat mogelijk en passend is, brengt verschillende partijen bij elkaar en bewaakt de voortgang. Een zelfstandige en afwisselende functie waarin je veel onderweg bent en bijdraagt aan de verdere ontwikkeling van onze werkwijze. Over Fokus Fokus maakt vrij en onafhankelijk wonen mogelijk voor mensen met een fysieke beperking. Verspreid over Nederland bieden we ADL-assistentie aan mensen die zelfstandig wonen in een aangepaste Fokuswoning. Eigen regie staat daarbij centraal: cliënten bepalen zelf wanneer en hoe zij assistentie ontvangen. Jouw rol In deze veelzijdige functie, die in ontwikkeling is, zorg je voor een zo efficiënt mogelijke in-, door- en uitstroomproces van cliënten. Samen met je collega's en ondersteunende diensten zorg je dat zowel de cliënt als overige betrokkenen tevreden zijn en de woningen zo kort mogelijk leeg staan. Met je vakkennis en communicatieve vaardigheden breng je rust, duidelijkheid en zorg je voor snelheid in het proces. Elke werkdag is anders waarbij je afwisselend onderweg bent, thuis werkt of op 1 van onze kantoren. Wat ga je doen - Bij de instroom van cliënten onderzoek je samen met de afdeling Servicepunt de dossiers van kandidaten. - Je gaat op huisbezoek en spreekt met verschillende partijen. - Je onderhoudt contacten met kandidaten, managers en teamleiders. - Bij de doorstroom van cliënten bekijk je samen met interne partijen of de cliënt kan doorverhuizen. - Je onderzoekt bij de uitstroom van cliënten, samen met interne partijen, wat de mogelijkheden zijn.
