Senior Jeugdconsulent Doesburg Werken voor Kamphorst Detachering is werken aan het verbeteren van de publieke sector in brede zin. Jouw taken en verantwoordelijkheden Het zelfstandig uitvoeren van integraal onderzoek, het opstellen van onderbouwde ondersteuningsplannen en het voeren van regie op ingezette jeugdhulp. De senior opdrachtnemer neemt een volledige caseload over en draagt verantwoordelijkheid voor het analyseren, begeleiden en monitoren van deze dossiers. De nadruk ligt op complexe casuïstiek, risicotaxatie en het organiseren van passende ondersteuning. Werkzaamheden - Voeren van (gezins)gesprekken, veelal op locatie, om de daadwerkelijke hulpvraag scherp te krijgen; - Uitvoeren van breed, integraal onderzoek en opstellen van volledig onderbouwde verslagen en ondersteuningsplannen; - Signaleren van veiligheidsrisico's en uitvoeren van passende acties, inclusief het maken van scherpe risico-inschattingen; - Zelfstandig oppakken en beheren van een toegewezen caseload, bestaande uit complexe en meervoudige problematiek; - Organiseren, monitoren en bijsturen van ingezette hulp, met focus op resultaat en tijdige interventies; - Normaliseren waar mogelijk, mobiliseren van het netwerk en inzetten van specialistische ondersteuning waar nodig; - Afstemmen met zorgaanbieders, scholen, gedragswetenschappers, Veilig Thuis en andere lokale partners; - Actief bijdragen aan een constructieve samenwerking binnen het team en aan de doorontwikkeling van werkprocessen; - Registreren en rapporteren in het regiesysteem MensCentraal.
