Wat je gaat doen - Je bent de schakel tussen het team, de behandelaar en de psychiater. - Je hebt een belangrijke rol in de coördinatie van zorg en richt je vooral op de medisch verpleegkundige kant. - Je houdt je samen met de psychiater bezig met medicatiemonitoring. - Je denkt mee, adviseert en coacht en maakt deel uit van zeer complexe (psychiatrische) casussen. - Je voert intakegesprekken, de somatische en metabole screening en je stapt in bij crisisinterventie. - Je geeft psycho-educatie. - De werktijden zijn regelmatig. Waar je gaat werken Het FACT team biedt behandeling aan jeugd en volwassenen met ernstig complexe gedrags- en psychiatrische stoornissen in combinatie met een licht verstandelijke beperking en bijkomende problematiek. Je maakt altijd deel uit van een multidisciplinair team, bestaande uit o.a. een psychiater, (GZ-)psycholoog/orthopedagoog generalist, casemanagers agogen/verpleegkundige en een ervaringsdeskundige. Wat breng jij mee - Je hebt je hbo SPV diploma. Je hebt een BIG-registratie. - Je bent betrouwbaar en flexibel en gaat moeilijke situaties niet uit de weg. - Je bent in staat om evenwicht te vinden in professionele nabijheid en kunt handelen in stressvolle situaties. - Je hecht veel waarde aan persoonlijk contact met cliënten. - Netwerken is voor jou geen probleem. - Je bent in het bezit van een rijbewijs en auto. Veel in de auto zitten vind je niet erg. Wat je krijgt - Een vast dienstverband voor minimaal 28-36 uur per week, uren verder in overleg. - Jouw werktijden zijn regelmatig.
