Werken met een gevarieerd takenpakket waarbij de cliënt centraal staat. Je werkt daarbij als SPV met complexe zorgvraagstukken in een afwissende omgeving. GGZ Rivierduinen zoekt voor de poli volwassenen in Gouda professionals die graag bijdragen aan het continue verbeteren van ons zorgaanbod. Voor de polikliniek volwassenenpsychiatrie in Gouda zoeken wij een nieuwe collega SPV. Binnen onze polikliniek speel je een belangrijke rol in de opvang en behandeling van cliënten met o.a. SAS (stemmings-, angst- en somatoforme stoornissen) en van Persoonlijkheidsproblematiek. Wanneer cliënten, vaak in crisis, bij ons binnen komen ben jij hun eerste zekerheid. We zoeken daarom een nieuwe collega SPV die zijn/haar deskundigheid in wil zetten voor onze cliënten. Binnen ons multidisciplinaire team draag je bij aan de diagnose en het opstellen van herstelplannen in samenspraak met de regiebehandelaar. Als SPV geef jij de input op basis van jouw waarnemingen van de patiënt. Dit doe je door: - Begeleiden en behandelen van je cliënten en hun systeem zowel individueel als in groepen; - Verrichten van intakes; - Actief deelnemen aan behandelplan- en casuïstiekbesprekingen; - Contact hebben met familie en/of naasten van de cliënten; - Onderhouden van je contacten met ketenpartners; - Coördineren van de zorg en behandeling; - Het werken in crisisdiensten.
